Right-sided pain?
Hello, doctor.
I have been experiencing severe pain on the right side of my body for the past two days.
The pain is located in the following areas: near the spine in the lower back, below the rib cage, above the hip, in the front of the thigh, and on the outer side of the knee.
Heat therapy has not provided relief, and today is a holiday, so I am hesitant to take pain medication on my own.
I have an autoimmune disease and have been taking quinine and corticosteroids (prednisolone) for seven months.
I would like to ask which department I should visit at the hospital for this condition.
Thank you.

When experiencing severe right-sided pain, especially in the areas you described—near the spine, under the ribs, at the hip, and down to the thigh and knee—it is crucial to seek medical attention promptly. Given your history of an autoimmune disease and the medications you are currently taking, including quinine and prednisolone, there are several considerations to keep in mind.
Firstly, the sudden onset of severe pain can indicate a variety of underlying issues, some of which may require immediate medical intervention. Conditions such as a herniated disc, kidney stones, or even an abdominal issue like appendicitis or gallbladder disease could present with similar pain patterns. Additionally, your use of corticosteroids like prednisolone can affect your immune response and may predispose you to certain infections or complications, which could exacerbate pain.
Given the complexity of your symptoms and medical history, it would be advisable to visit an emergency department or urgent care facility. When you arrive, you should inform the medical staff about your autoimmune condition and the medications you are taking, as this information will guide their assessment and treatment plan. They may perform imaging studies, such as an X-ray or MRI, to rule out fractures, disc issues, or other structural problems.
In terms of which specialty to consult, starting with either an emergency medicine physician or an orthopedic specialist would be appropriate. If the pain is suspected to be related to your abdominal organs, a general surgeon or gastroenterologist may also be involved in your care.
While waiting for medical attention, it is understandable that you may want to manage your pain. However, since you mentioned that heat therapy has not provided relief and you are hesitant to take over-the-counter pain medications, it is best to avoid self-medicating without professional guidance, especially considering your current medications.
In summary, the sudden and severe nature of your pain, combined with your medical history, warrants immediate evaluation by a healthcare professional. Do not hesitate to seek help, as timely intervention can be crucial in managing potential complications and ensuring your well-being.
