Post-Hemorrhoid Surgery: Is Frequent Bleeding Normal After 9 Months? - Surgery

Nine months after hemorrhoid surgery (electrosurgery)?

It has been nine months since my hemorrhoid surgery, but since the wound healed, I have been experiencing a very high frequency of bleeding during bowel movements (at least three times a week).
The blood is bright red and there is no other pain.
I always notice bleeding when I wash my buttocks with warm water.
Is this situation abnormal? Do I need to undergo further examination?

Manman, 30~39 year old female. Ask Date: 2017/01/04

Dr. Ke Fangxu reply Surgery

If it has been nine months since the hemorrhoid surgery, the current bleeding is likely related to constipation or prolonged straining during bowel movements.
If the bleeding persists, it is necessary to seek medical treatment or have the surgeon evaluate the situation.

More Info

After undergoing hemorrhoid surgery, experiencing frequent bleeding during bowel movements, especially nine months post-operation, is concerning and warrants further evaluation. While some bleeding can be expected after such procedures, the frequency and amount you describe—at least three times a week with bright red blood—suggest that there may be an underlying issue that needs to be addressed.

In the initial weeks following hemorrhoid surgery, it is common for patients to experience some bleeding as the surgical site heals. However, as time progresses, the frequency and severity of bleeding should decrease significantly. If you are still experiencing bleeding at this stage, it could indicate several potential complications:
1. Incomplete Healing: The surgical site may not have healed properly, leading to ongoing irritation and bleeding. This could be due to factors such as excessive straining during bowel movements, which can disrupt the healing process.

2. Recurrence of Hemorrhoids: It is possible for hemorrhoids to recur even after surgical intervention. If new hemorrhoids have developed, they may cause similar symptoms to those experienced prior to surgery.

3. Anal Fissures: The presence of anal fissures, which are small tears in the lining of the anus, can also lead to bright red bleeding during bowel movements. These can occur due to hard stools or straining.

4. Other Conditions: There are other potential causes of rectal bleeding, including diverticular disease, colorectal polyps, or even more serious conditions such as colorectal cancer. While these are less common, they should not be ruled out, especially if the bleeding persists.

Given your symptoms, it is advisable to consult with your healthcare provider or a specialist in gastroenterology or colorectal surgery. They may recommend a thorough examination, which could include a digital rectal exam, anoscopy, or colonoscopy, to visualize the area and determine the cause of the bleeding.

In terms of management, your doctor may suggest dietary changes to ensure softer stools, such as increasing fiber intake and hydration. They may also recommend stool softeners or laxatives if necessary. If an underlying condition is identified, appropriate treatment will be initiated based on the diagnosis.

Regarding your concern about the need for further checks, it is crucial to address any ongoing bleeding, as it can lead to complications such as anemia if significant blood loss occurs over time. Therefore, do not hesitate to seek medical attention to ensure your health and peace of mind.

In summary, while some bleeding can be normal after hemorrhoid surgery, the frequency and amount you are experiencing nine months post-operation are not typical. It is essential to consult with your healthcare provider to investigate the cause and receive appropriate treatment.
