Blood in Semen: Causes and When to Seek Medical Help - Urology

Hematospermia

About three years ago, while riding as a passenger on a motorcycle, I experienced a sudden stop (at a speed of approximately 20 km/h) and felt a jolt in my perineum when I hit the seat.
Since then, I have noticed that during masturbation, my semen sometimes appears to have black strands.
Initially, I thought it was lint from my clothing, but later I realized it might be blood.
Just as I was planning to see a doctor, this symptom disappeared.
It occasionally reappeared but then went away again.
Recently, I have noticed this symptom again.
Should I seek medical attention? What could be the possible issue? I have previously been diagnosed with varicocele, and at that time, the doctor mentioned it was severe.
However, after a few years, another doctor examined me and said that the findings were actually quite normal, with only a slight issue.

Dr. Du Shixi reply Urology

Hello: It is likely that there is damage to the microvasculature in the perineum, which could be causing the bleeding.
However, this is just a hypothesis.
I recommend that you consult a urologist for an examination.
In the meantime, please avoid vigorous exercise, alcohol, and spicy or irritating foods.
Thank you for your inquiry.

Blood in semen, also known as hematospermia, can be a concerning symptom for many individuals. It can arise from various causes, some of which are benign, while others may require medical attention. Given your history of experiencing blood in your semen intermittently, particularly after a traumatic event, it is essential to understand the potential causes and when to seek medical help.

Potential Causes of Blood in Semen
1. Trauma: As you mentioned, the incident of hitting your perineum while riding a bike could have caused minor trauma to the reproductive organs, leading to bleeding. Trauma can cause bruising or damage to blood vessels, resulting in blood appearing in the semen.

2. Infections: Infections of the prostate (prostatitis), seminal vesicles, or urethra can lead to inflammation and bleeding. Symptoms may include pain during ejaculation, frequent urination, or discomfort in the pelvic area.

3. Vascular Issues: You noted a previous diagnosis of varicocele (enlarged veins within the scrotum). Varicoceles can sometimes lead to blood in the semen due to increased pressure in the veins, which may affect blood flow and lead to bleeding.

4. Benign Prostatic Hyperplasia (BPH): In older men, an enlarged prostate can cause bleeding, especially if there is associated inflammation.

5. Tumors: Although less common, tumors in the prostate, seminal vesicles, or other parts of the reproductive system can cause hematospermia. This is particularly concerning if accompanied by other symptoms such as weight loss, pain, or changes in urination.

6. Systemic Conditions: Conditions such as hypertension or clotting disorders can also lead to bleeding in various parts of the body, including the reproductive system.

When to Seek Medical Help
You should consider seeking medical attention if you experience any of the following:
- Persistent blood in semen over multiple ejaculations.

- Accompanying symptoms such as pain during ejaculation, pelvic pain, fever, or changes in urination.

- A significant increase in the amount of blood or if the blood appears bright red, which may indicate active bleeding.

- If you have a history of trauma and the symptoms do not resolve or worsen.

Recommended Actions
1. Consult a Urologist: Given your history of varicocele and the recent symptoms, it would be prudent to consult a urologist. They can perform a thorough examination, including a physical exam and possibly imaging studies, to assess the condition of your reproductive organs.

2. Keep a Symptom Diary: Documenting when the blood appears, any associated symptoms, and any activities that may correlate with the bleeding can provide valuable information to your healthcare provider.

3. Avoid Self-Diagnosis: While it is tempting to attribute the symptoms to past trauma or a benign cause, it is essential to have a professional evaluation to rule out more serious conditions.

4. Follow-Up: If you notice that the symptoms resolve but then recur, it is still advisable to follow up with a healthcare provider to ensure that there are no underlying issues.

In conclusion, while blood in semen can sometimes be benign, it is essential to approach this symptom with caution, especially given your medical history. Seeking medical advice will help ensure that any potential issues are addressed promptly and appropriately.
