Menstrual issues

Hello Doctor, my last menstrual period was on December 16, and the one before that was on November 20.
Both of these months had a 25-day cycle, while my previous cycles were 30 days.
This month, my period was late, and I noticed a little bit of pink discharge yesterday, January 13.
I've been experiencing some cramping in my abdomen, and this morning the discharge turned brown, only visible when I wipe; there was nothing on the pad.
I haven't used any contraception in the past few months as I am trying to conceive my second child.
We have been intimate 2-3 times a week.
However, I had a salpingectomy and oophorectomy due to an ectopic pregnancy on July 5.
Could this be causing my irregular periods, or is it possible that I could be pregnant?

Niming, 20~29 year old female. Ask Date: 2017/01/14

Hello, if you suspect you might be pregnant, it's advisable to wait for a chance to confirm.
If your period is more than a week late, it is recommended to see a gynecologist.
Wishing you good health.

Irregular periods can be a source of concern for many women, especially when trying to conceive. Based on your description, it seems you are experiencing some changes in your menstrual cycle, which can be influenced by various factors, including recent medical history, hormonal fluctuations, and potential pregnancy.

Firstly, it’s important to understand that menstrual cycles can vary significantly from month to month. A typical cycle ranges from 21 to 35 days, and it is not uncommon for women to experience irregularities, especially after significant medical events such as surgery. In your case, having undergone a salpingectomy (removal of the fallopian tube) due to an ectopic pregnancy can indeed impact your menstrual cycle. The body may take some time to adjust hormonally after such a procedure, leading to variations in cycle length and flow.

Regarding your recent symptoms—light pink spotting followed by brown discharge—this could be indicative of several possibilities. One common explanation for light spotting is implantation bleeding, which can occur when a fertilized egg attaches to the uterine lining. This typically happens around the time your period is due, which could align with your desire to conceive. However, it is also possible that the spotting is related to hormonal fluctuations or residual effects from your previous surgery.

If you are actively trying to conceive, it is advisable to take a home pregnancy test if your period does not start as expected. Home pregnancy tests are generally reliable and can detect pregnancy hormones (hCG) in your urine about a week after a missed period. If the test is negative and your symptoms persist, or if you have concerns about your cycle, it would be prudent to consult with a healthcare provider. They may recommend blood tests to measure hCG levels or perform an ultrasound to assess your reproductive health.

In addition to potential pregnancy, other factors that can lead to irregular periods include stress, significant weight changes, excessive exercise, and hormonal imbalances. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can also contribute to irregular cycles. Given your history of ectopic pregnancy and surgery, it is essential to monitor your symptoms closely.

As for your question about whether you could be pregnant, it is certainly a possibility, especially since you have not been using contraception and have been sexually active. However, the only way to confirm this is through a pregnancy test or consultation with a healthcare provider.

In summary, while irregular periods can be concerning, they are often a normal part of reproductive health, especially after surgical interventions. If you suspect pregnancy, taking a test and consulting with your doctor for further evaluation is the best course of action. They can provide personalized advice based on your medical history and current symptoms. Remember to keep track of your cycle and any changes you experience, as this information can be valuable for your healthcare provider in assessing your reproductive health.
