Hernia recurrence and testicular pain?

Hello doctor, I previously had a hernia on the left side with one testicle larger than the other.
I underwent surgery in June last year for "tension-free repair using autologous tissue." After the surgery, my left testicle has been somewhat painful, and it still appears to be higher than the right one.
The doctor mentioned that it would improve over time, but even now, the testicles remain uneven.
After ejaculation, the left testicle still experiences pain.
I have also experienced a few instances of bowel descent on both the left and right sides, but most of the time, everything seems normal.
Could this be considered a recurrence? Can a diagnosis be made through physical examination?

Dear Mr.
Hou:
1.
If you still have a hernia with intestinal protrusion, please consult a nearby surgeon.

2.
If there is a size discrepancy between the testicles, it may indicate a possible intestinal issue; please have a doctor evaluate the treatment options.

After undergoing surgery for a left inguinal hernia, it is understandable to have concerns regarding postoperative pain, especially in the testicular area, and whether this indicates a recurrence of the hernia. Your description of experiencing pain in the left testicle, along with the sensation of one testicle being higher than the other, raises several important considerations.

First, it is essential to understand that postoperative pain can be a common occurrence following hernia repair surgery. This pain may stem from several factors, including nerve irritation, inflammation, or even muscle strain in the surgical area. The surgical procedure itself can lead to changes in the anatomy and function of the surrounding tissues, which may contribute to discomfort. In many cases, this pain diminishes over time as the body heals.

Regarding the asymmetry of the testicles, it is important to note that slight differences in testicular position are not uncommon and can be a normal anatomical variation. However, if the difference is significant or accompanied by pain, it warrants further investigation. The pain you describe, particularly after ejaculation, could be related to several factors, including nerve irritation or tension in the surrounding muscles and tissues.

The concern about hernia recurrence is valid, especially if you are experiencing symptoms such as a sensation of the intestines "dropping" or bulging in the area. Recurrence can occur, particularly if the repair was not entirely successful or if there was excessive strain on the surgical site during recovery. However, the absence of a visible bulge or protrusion at the surgical site is a positive sign, suggesting that a recurrence may not be present.

To determine whether a recurrence has occurred, a physical examination by a healthcare professional is crucial. During this examination, the doctor will assess the surgical site for any signs of bulging or abnormality. They may also perform a testicular examination to evaluate the position and condition of the testicles. Imaging studies, such as an ultrasound, may be utilized if there is a suspicion of complications or if the physical examination findings are inconclusive.

In the meantime, it is advisable to monitor your symptoms closely. If the pain persists, worsens, or is accompanied by other concerning symptoms such as swelling, redness, or fever, you should seek medical attention promptly. Additionally, if you have been engaging in activities that involve heavy lifting or straining, it may be beneficial to modify these activities to reduce strain on the surgical site.

In summary, while postoperative pain and testicular asymmetry can be concerning, they do not necessarily indicate a hernia recurrence. A thorough evaluation by a healthcare professional is essential to determine the underlying cause of your symptoms and to ensure appropriate management. If you have not already done so, consider scheduling a follow-up appointment with your surgeon or a urologist to discuss your symptoms and receive personalized care.
