Postoperative issues?

Hello Doctor: In late August 2016, I had four stitches on my heel due to an injury (similar to a laceration).
As of now, I believe the wound has healed well, but the only issue is that a few months ago, when I took hot showers or exercised, I sometimes felt itchiness at the wound site (the surface of the wound has completely healed, and I have not used any cosmetic gels or other products, allowing the wound to heal naturally; my shower lasts about 10 minutes).
However, recently, when taking hot showers, the wound has become itchier than before, and sometimes I even experience a slight stinging sensation.
Is this situation normal? Thank you, Doctor, for your assistance!

Post-surgery itching and discomfort can be a common experience for many patients, and it is essential to understand the underlying reasons for these sensations. In your case, having undergone a minor surgical procedure on your heel in August 2016, it is not unusual to experience itching or discomfort even several months after the wound has healed.
Itching at the surgical site can occur for several reasons. First, as the skin heals, it undergoes a process of regeneration that can lead to sensations of itching. This is often due to the nerve endings in the skin being stimulated as they recover. Additionally, the skin may become dry or irritated, especially if exposed to hot water or certain soaps, which can exacerbate the itching sensation.
The fact that you mentioned the itching has increased with hot showers suggests that the heat may be causing the blood vessels to dilate, leading to increased blood flow to the area and potentially heightening the sensation of itching. It is also possible that the heat is causing the skin to dry out more quickly, which can lead to further irritation and discomfort.

The presence of a stinging or sharp pain, in conjunction with the itching, may indicate that the nerve endings are still quite sensitive in that area. This sensitivity can persist for months or even years after surgery, particularly if the incision was deep or if there was significant tissue trauma during the procedure.
While these sensations can be normal, it is crucial to monitor them. If the itching becomes unbearable, or if you notice any signs of infection (such as increased redness, swelling, warmth, or discharge), it is essential to seek medical attention. Infections can sometimes present with itching, and it is better to err on the side of caution.

To manage the itching, consider the following recommendations:
1. Moisturize: Applying a gentle, fragrance-free moisturizer to the area can help alleviate dryness and reduce itching. Avoid any products that contain alcohol or strong fragrances, as these can irritate the skin further.

2. Cool Compress: Applying a cool compress to the area can help soothe the itching sensation. This can be particularly effective after hot showers.

3. Avoid Hot Water: If hot water triggers the itching, consider using lukewarm water for showers and limit the duration of exposure.

4. Gentle Cleansing: Use mild, hypoallergenic soap when washing the area to minimize irritation.

5. Avoid Scratching: It can be tempting to scratch the area, but this can lead to further irritation or even break the skin, increasing the risk of infection.

6. Consult a Dermatologist: If the itching persists or worsens, consulting a dermatologist may be beneficial. They can assess the area and may recommend topical treatments, such as hydrocortisone cream, to help alleviate the itching.

In summary, while post-surgery itching and discomfort can be a normal part of the healing process, it is essential to remain vigilant and seek medical advice if you have any concerns. Proper care and attention to the affected area can help manage these sensations effectively.
