Persistent Cloudy Urine for Two Years: Possible Causes and Concerns - Urology

Share to:

The urine has been cloudy for two years and has recently worsened, but tests conducted a year ago showed no issues?

Hello, doctor.
Thank you very much for your patient response regarding my cloudy urine issue, which has persisted for over two years.
The frequency is about 80% cloudy and 20% clear.
A year ago, I went to the hospital for an examination, but the report indicated no issues (the sample tested at that time happened to be clear).
Recently, while studying in the United States, I have been unable to return to Taiwan for further testing.
However, the cloudiness has become increasingly severe, to the point where it resembles ink or flour being poured, with visible white particles spreading.
Sometimes, the urine stream is noticeably cloudy, and after urination, I can see a milky fluid containing suspended white particles coming from the urethra.
Occasionally, after wearing tighter pants for a whole day, the cloudiness of my urine worsens.
Other symptoms include a foul odor in my urine: this has occurred once or twice in the past few days but then subsided.
I experience occasional burning pain in the urethra, but the frequency is very low.
After ejaculation, I often feel a sense of testicular heaviness or pain, but this occurs very rarely.
I have taken my temperature and have not had a fever, but I frequently feel fatigued and experience skin and muscle tenderness (similar to the discomfort of not wanting to be touched when having a fever).
Tapping on my back in the kidney area does not cause pain (my father mentioned that this would hurt when he had pyelonephritis).
Currently, I cannot return to Taiwan in the short term, and I am very worried about whether I can hold out until I can get checked.
I am concerned that there may be an issue with my health.
Thank you again, doctor, for your patient response.

Hello: If your urine is cloudy and drinking 2000cc of water a day does not improve the situation, you should seek medical attention.
If there is occasional discharge from the urethral opening, it may indicate a bladder or urethral issue; if not, it could be related to the kidneys.
It is recommended to bring a sample of the problematic urine (do not let it sit for more than ten minutes) for the doctor to examine.
If there are kidney issues, do not delay and seek medical care as soon as possible.
Thank you for your inquiry.

Persistent cloudy urine can be a concerning symptom, especially when it has lasted for two years. The fact that your urine is frequently cloudy, with a notable increase in turbidity and the presence of white particles, suggests that there may be underlying issues that need to be addressed. Here are some potential causes and considerations based on your symptoms:
1. Urinary Tract Infection (UTI): One of the most common causes of cloudy urine is a urinary tract infection. Symptoms can include cloudy urine, a strong odor, and sometimes burning sensations during urination. While you mentioned that the burning sensation is infrequent, the presence of a foul smell could indicate an infection. Even if you do not have pain when tapping on your kidneys, it does not rule out a UTI.

2. Dehydration: Cloudy urine can also result from dehydration, which concentrates the urine and can lead to a more opaque appearance. Ensure you are drinking enough fluids, especially if you are in a hot climate or are physically active.

3. Kidney Stones: The presence of white particles in your urine could suggest the presence of kidney stones or crystals. These can cause irritation in the urinary tract and may lead to cloudy urine. If you experience any severe pain, especially in the lower back or sides, this could be a sign of kidney stones.

4. Prostatitis: Given your mention of occasional testicular pain and the fact that you are male, prostatitis (inflammation of the prostate gland) could be a consideration. This condition can lead to urinary symptoms, including cloudy urine and discomfort.

5. Dietary Factors: Certain foods and supplements can affect urine clarity. For example, high protein diets, certain vitamins, and foods like asparagus can alter the appearance and smell of urine.

6. Sexually Transmitted Infections (STIs): Some STIs can cause changes in urine, including cloudiness. If you are sexually active, it may be worth considering this as a potential cause.

7. Systemic Conditions: Conditions such as diabetes can lead to changes in urine appearance. If you are feeling fatigued and have other systemic symptoms, it may be worth discussing with a healthcare provider.

Given the duration of your symptoms and the changes you are experiencing, it is crucial to seek medical attention as soon as possible. While you may not be able to return to Taiwan immediately, consider visiting a local healthcare provider in the U.S. They can perform a urinalysis and possibly a culture to determine if there is an infection or other underlying issue.
In the meantime, keep track of any additional symptoms, such as fever, increased pain, or changes in your overall health, as these can provide important information to your healthcare provider. Avoid tight clothing that may irritate the area, and ensure you are staying well-hydrated.
In summary, while there are several potential causes for your symptoms, it is essential to have a thorough evaluation to determine the underlying issue and appropriate treatment. Do not hesitate to seek care, as early intervention can prevent complications and improve your overall health.
