Red rashes on the glans and urethra that fluctuate in severity and do not improve?
Hello, Doctor.
About six months ago, I noticed some red rashes and small abrasions on the surface of the glans and near the coronal sulcus.
They are not painful or itchy, and I can see some reddened small wounds at the urethral opening.
There is a very mild burning sensation when urinating, but these symptoms disappear naturally after a few days.
It looks like they have healed, but after a while, they flare up again, and this cycle of improvement and recurrence has continued for about six months.
Last month, I finally went to a urology department at a large hospital, where the doctor examined my genital area.
The doctor diagnosed it as eczema and prescribed topical medication.
My urine test results were normal, but after using the medication for almost a month, the symptoms are still the same, with periods of improvement and recurrence.
Could this be urethritis or balanitis? Why can't it be completely cured? I usually drink 2000cc of water daily, do not hold my urine, always clean under the foreskin, and have a habit of jogging and exercising.
My diet includes fruits and vegetables.
Aside from masturbation, I have no sexual activity.
If I have any shortcomings, it would be that I tend to sleep late (after midnight) and occasionally eat junk food.
I consider my lifestyle to be relatively healthy, so why is this happening? I kindly ask for your clarification.
Thank you.

Hello: Eczema is difficult to completely resolve; when immunity decreases, localized dampness and heat can lead to a recurrence.
Based on your description, take advantage of the current cold weather to keep the area dry.
When showering, use only clean water and avoid soap or body wash.
After washing, dry the area with a clean, soft cloth.
If the foreskin is long, make sure to dry it thoroughly after urination to avoid moisture.
If there are any broken areas, it is advisable to see a doctor to check for herpes.
Additionally, ensure you get at least 6 hours of sleep.
Give this a try.

More Info
Recurring red rashes on the glans and urethra can be concerning and uncomfortable. Based on your description, it sounds like you have been experiencing symptoms consistent with a dermatological condition, possibly eczema or dermatitis, as suggested by the urologist you consulted. However, the intermittent nature of your symptoms and the fact that they have not fully resolved with treatment warrant further exploration.
Possible Causes
1. Eczema or Dermatitis: This is a common skin condition that can affect the genital area. It can be triggered by irritants (like soaps, detergents, or even sweat) or allergens (like certain fabrics or personal care products). The fact that your symptoms are not painful or itchy but appear as red patches aligns with this diagnosis.
2. Infections: While your urine tests came back normal, conditions like balanitis (inflammation of the glans) or urethritis (inflammation of the urethra) can sometimes present with similar symptoms. These conditions can be caused by fungal infections, bacterial infections, or sexually transmitted infections, even if you are not currently sexually active.
3. Allergic Reactions: An allergic reaction to products used in the genital area, such as soaps, lotions, or even latex from condoms, could also cause recurring rashes.
4. Skin Conditions: Other skin conditions, such as psoriasis or lichen planus, could also present with similar symptoms. These conditions may require different treatments than eczema or dermatitis.
Why It May Not Be Resolving
1. Inadequate Treatment: The topical medication prescribed may not be sufficient for your specific condition. Sometimes, stronger topical steroids or different classes of medications are necessary to manage persistent dermatitis.
2. Ongoing Irritation: If you are still exposed to irritants or allergens, the condition may continue to flare up. It’s important to identify and eliminate any potential triggers.
3. Underlying Conditions: There may be an underlying condition that has not yet been diagnosed. Conditions like diabetes or immune system disorders can sometimes manifest as skin issues.
Recommendations
1. Follow-Up with a Dermatologist: Since your symptoms have persisted despite treatment, it may be beneficial to see a dermatologist who specializes in skin conditions. They can provide a more thorough examination and possibly perform a skin biopsy if necessary.
2. Patch Testing: If an allergic reaction is suspected, patch testing can help identify specific allergens that may be causing your symptoms.
3. Review Hygiene Practices: While you mentioned maintaining good hygiene, consider reviewing the products you use for washing and grooming. Opt for fragrance-free and hypoallergenic products to minimize irritation.
4. Lifestyle Modifications: Continue to maintain a healthy lifestyle, but consider adjusting your sleep schedule and dietary habits. Poor sleep and a diet high in processed foods can impact your overall skin health.
5. Consider Stress Management: Stress can exacerbate skin conditions. Incorporating stress-reducing activities such as yoga, meditation, or regular exercise can be beneficial.
6. Document Symptoms: Keep a diary of your symptoms, noting any potential triggers, dietary changes, or new products used. This information can be invaluable for your healthcare provider in determining the cause of your symptoms.
In conclusion, while your symptoms may initially appear benign, their persistence and recurrence suggest that further evaluation is necessary. A multidisciplinary approach involving both urologists and dermatologists may provide the best chance for a definitive diagnosis and effective treatment.
