Can I get pregnant?
Hello Doctor.
My girlfriend's last menstrual period started on December 25.
We had unprotected intercourse on January 14 and January 21 (withdrawal method), and now her period has not arrived yet.
Recently, she has been experiencing some symptoms, such as breast tenderness, acne, frequent fatigue, and a lump in one breast that feels harder on one side and softer on the other, but both sides are painful.
These are the current symptoms.
She often stays up late and only drinks cold water.
Could these be the reasons for her symptoms?
Li, 10~19 year old female. Ask Date: 2017/01/31
Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
Additionally, early pregnancy does not always present specific symptoms, and subjective symptoms cannot be used to determine pregnancy.
It is recommended to take a pregnancy test if your period is more than a week late.
Thank you! Wishing you peace and health, Dr.
Chang.

It sounds like you are understandably concerned about your girlfriend's potential pregnancy given the timing of her last menstrual period and the sexual activity you described. Let's break down the situation and address your concerns.
First, the timing of her last menstrual period on December 25, combined with the sexual encounters on January 14 and January 21, places her in a window where pregnancy could be possible. Typically, ovulation occurs about 14 days before the next expected period, which means if her cycle is regular, she may have been ovulating around that time. Since her period has not arrived yet, this could be an indication of pregnancy, but it is not definitive.
The symptoms you mentioned—breast tenderness, acne, fatigue, and the presence of a lump in one breast—can be associated with premenstrual syndrome (PMS), hormonal changes, or pregnancy. Breast tenderness and changes in breast tissue can occur due to hormonal fluctuations, particularly if she is approaching her menstrual cycle or if she is pregnant. The lump you mentioned could be a benign condition such as a cyst or fibrocystic breast changes, which are common and often fluctuate with hormonal changes. However, it is essential for her to have any new or unusual breast lumps evaluated by a healthcare professional.
Additionally, lifestyle factors such as lack of sleep and poor hydration (only drinking cold water) can contribute to feelings of fatigue and may exacerbate hormonal imbalances. Stress and lifestyle choices can significantly impact menstrual cycles and overall health. If she is experiencing significant stress or has irregular sleep patterns, these could also be influencing her symptoms.
Given that her period is late and she is experiencing these symptoms, it would be advisable for her to take a home pregnancy test. These tests are generally reliable and can provide a quick answer. If the test is positive, she should schedule an appointment with a healthcare provider for confirmation and to discuss her options moving forward. If the test is negative but her period still does not arrive, it would be wise for her to consult with a healthcare provider to explore other potential causes for her symptoms and missed period.
In summary, while there is a possibility of pregnancy given the timing and symptoms, it is crucial for her to take a pregnancy test and consult with a healthcare professional for a thorough evaluation. This will help address her symptoms and provide clarity on her reproductive health. It’s important to approach this situation with care and support, as it can be a stressful time for both of you.
