Penile issues

Hello Doctor: I would like to ask about my condition.
My glans cannot be exposed whether I am erect or not (I have never seen my entire glans, only the meatus).
Some doctors have said it’s not a problem, but will it be very painful when I have intercourse in the future? My foreskin gets stuck and painful if I pull it back less than an inch.
During intercourse, won’t I be unable to insert fully? Could it cause bleeding?

A, 10~19 year old female. Ask Date: 2017/02/13

Hello: The glans cannot be exposed during both erection and flaccidity, which is a phenomenon of phimosis.
The doctor may recommend a circumcision.
However, I have seen many patients with this condition who have multiple children and do not have issues with sexual activity.
Therefore, you can consult another physician for a second opinion.
However, it is not an emergency procedure, so there is no need to be overly anxious.
If surgery is chosen, it is better to schedule it before a holiday for easier post-operative care.
In the meantime, just maintain cleanliness and dryness.
Thank you for your inquiry.

Phimosis, the condition where the foreskin cannot be easily retracted over the glans (the head of the penis), can lead to various concerns regarding sexual health and comfort during sexual activity. It is essential to address your worries about potential pain during intercourse, the risk of bleeding, and the overall implications for your sexual health.

Firstly, it is important to understand that phimosis can be classified into two types: physiological and pathological. Physiological phimosis is common in infants and young boys, where the foreskin is naturally non-retractable due to developmental reasons. In most cases, this condition resolves itself as the child grows older. Pathological phimosis, on the other hand, occurs when the foreskin becomes tight due to scarring, inflammation, or infection, making retraction painful or impossible.

Your concern about pain during sexual intercourse is valid. If the foreskin is tight and cannot be retracted, attempting to engage in penetrative sex may indeed cause discomfort or pain. The tightness can lead to tearing of the foreskin or even bleeding if there is excessive force applied. This is particularly concerning if you have already experienced pain when trying to retract the foreskin even slightly.
Moreover, if the foreskin is unable to retract, it can lead to other complications, such as recurrent infections or inflammation (balanitis), which can further exacerbate discomfort during sexual activity. In some cases, the inability to retract the foreskin can also lead to a condition called paraphimosis, where the retracted foreskin cannot return to its original position, leading to swelling and pain.

To address your concerns, it is advisable to consult with a urologist or a healthcare professional who specializes in male sexual health. They can perform a thorough examination and provide guidance tailored to your specific situation. Treatment options may include:
1. Topical Steroids: In some cases, applying a topical steroid cream can help loosen the tight foreskin and make retraction easier over time.

2. Gentle Stretching Exercises: Under the guidance of a healthcare professional, you may be advised to perform gentle stretching exercises to gradually increase the foreskin's elasticity.

3. Circumcision: If phimosis is severe and causing significant discomfort or recurrent infections, circumcision (surgical removal of the foreskin) may be recommended. This procedure can eliminate the problem entirely and improve sexual function.

4. Preputioplasty: This is a less invasive surgical option that involves widening the opening of the foreskin without complete removal. It may be suitable for some individuals with phimosis.

In the meantime, it is crucial to avoid any sexual activities that may cause pain or discomfort. Engaging in sexual intercourse with a tight foreskin can lead to injury and further complications. Open communication with your partner about your condition is also essential, as it can help alleviate anxiety and foster understanding.

Lastly, it is important to maintain good hygiene, especially if you are unable to retract the foreskin. Regular cleaning of the area can help prevent infections and other complications. If you notice any unusual symptoms, such as swelling, redness, or discharge, seek medical attention promptly.

In summary, while phimosis can pose challenges to sexual health, there are effective treatment options available. Consulting with a healthcare professional will provide you with the best course of action to ensure your comfort and well-being during sexual activity.
