Sudden onset chest tightness?

Hello Doctor: My mother is 45 years old and has no history of heart disease.
Yesterday, she suddenly experienced chest tightness with a feeling of pressure, as if her chest was tightly bound, which lasted all day.
Today, along with the chest tightness, she also experienced shortness of breath, cold sweats, and dizziness for about half an hour.
After resting quietly, the accompanying symptoms eased, but she still has a persistent feeling of chest tightness and pressure throughout the day.
I am very worried about my mother's condition.
Thank you very much for your guidance.

Dr. Yang Wenda reply Pulmonology

Hello: Although there are many causes of chest tightness, if it persists for a long time, it is important to investigate the possible reasons.
It is recommended to first see a cardiologist to rule out heart disease, and then consult a pulmonologist.
Best wishes!

Sudden chest tightness can be a concerning symptom, especially when accompanied by other signs such as difficulty breathing, sweating, and dizziness. In your mother's case, the combination of these symptoms raises several important considerations that should not be overlooked.
Firstly, it is essential to understand that chest tightness can stem from various causes, ranging from benign to serious. Given that your mother is 45 years old and has no known history of heart disease, it is still crucial to rule out any cardiac issues. Conditions such as angina or even a heart attack can present with chest tightness, pressure, and associated symptoms like sweating and dizziness. Although she may not have a prior history of heart problems, risk factors such as high blood pressure, high cholesterol, diabetes, smoking, or a family history of heart disease can increase the likelihood of cardiac events.

Additionally, other potential causes of chest tightness include anxiety or panic attacks, which can manifest with physical symptoms similar to those of a heart condition. Anxiety can lead to hyperventilation, which might explain the breathing difficulties and dizziness. Gastroesophageal reflux disease (GERD) can also cause chest discomfort, often mistaken for heart-related issues, especially if there is a history of acid reflux or heartburn.

Given the symptoms your mother experienced, it is advisable to seek medical attention promptly. A healthcare professional can perform a thorough evaluation, including a physical examination, an electrocardiogram (ECG), and possibly blood tests to check for markers of heart damage. If cardiac causes are ruled out, further investigations may be warranted to explore other potential causes, such as anxiety disorders or gastrointestinal issues.

In the meantime, it is essential to monitor her symptoms closely. If she experiences any worsening of her condition, such as increased chest pain, prolonged difficulty breathing, or any new symptoms like pain radiating to the arm, jaw, or back, immediate medical attention should be sought.
Moreover, lifestyle factors can play a significant role in managing and preventing future episodes of chest tightness. Encouraging your mother to adopt a heart-healthy lifestyle, including regular physical activity, a balanced diet low in saturated fats and high in fruits and vegetables, and stress management techniques, can be beneficial. If anxiety is a contributing factor, exploring relaxation techniques such as deep breathing exercises, yoga, or mindfulness can also help alleviate symptoms.

In summary, while sudden chest tightness can be alarming, it is essential to approach the situation with a comprehensive understanding of potential causes. Seeking medical evaluation is crucial to ensure that any serious conditions are addressed promptly. Your concern for your mother's health is commendable, and taking proactive steps to understand and manage her symptoms will contribute to her overall well-being.
