Numbness in the genital area persists even after undergoing surgery for a herniated disc?
About five or six years ago, I underwent spinal disc surgery at Mackay Memorial Hospital.
After the surgery, I noticed that my genital area felt numb.
Initially, I thought it was due to the anesthesia not wearing off, but even a few days after being discharged, the numbness persisted.
I returned to the hospital for a follow-up, and the doctor explained that it was due to prolonged nerve compression.
He prescribed Vitamin B12, noting that my condition was peripheral neuropathy.
However, after taking the medication for several months, there seemed to be no improvement.
The doctor then suggested physical therapy with electrical stimulation to see if it would help.
After undergoing therapy for a month or two, there still appeared to be no improvement.
The physical therapist advised me to continue with the therapy, assuring me that it would eventually get better.
Although my genital area remains numb, I can still function normally; however, the numbness persists.
I would like to ask if there are other treatment options available, or should I continue with electrical stimulation therapy until it resolves?

Aside from medication, the only other option is radiation therapy; there are no other methods.

More Info
Persistent genital numbness following disc surgery can be a distressing and complex issue. It is understandable that you are seeking solutions after experiencing this sensation for several years. Based on your description, it appears that the numbness may be related to nerve compression that occurred prior to your surgery, which may have led to nerve damage or dysfunction. Here, I will outline potential avenues for treatment and management, as well as provide some insights into the underlying mechanisms of your symptoms.
Understanding the Condition
After disc surgery, particularly in the lumbar region, it is not uncommon for patients to experience various neurological symptoms, including numbness, tingling, or weakness in the lower extremities and pelvic region. These symptoms can arise from several factors:
1. Nerve Compression: If the nerve roots were compressed prior to surgery, they may have sustained damage that does not resolve immediately after the decompression. This can lead to persistent symptoms even after the offending disc material has been removed.
2. Neuropathy: Prolonged nerve compression can lead to neuropathy, which is characterized by damage to the peripheral nerves. This can result in sensations such as numbness or tingling.
3. Post-Surgical Changes: Scar tissue formation after surgery can also contribute to nerve irritation or compression, potentially leading to ongoing symptoms.
4. Psychological Factors: Chronic pain and numbness can also have psychological components, where anxiety or depression may exacerbate the perception of pain or discomfort.
Treatment Options
Given that you have already tried vitamin B12 supplementation and physical therapy without significant improvement, here are some additional treatment options you might consider:
1. Neurology Consultation: It may be beneficial to consult a neurologist who specializes in nerve disorders. They can perform a thorough evaluation, including nerve conduction studies and electromyography (EMG), to assess the function of the affected nerves and determine the extent of any damage.
2. Pain Management: A pain management specialist can offer various treatments, including medications such as gabapentin or pregabalin, which are often effective for neuropathic pain. They may also consider nerve blocks or other interventional procedures to alleviate symptoms.
3. Physical Therapy: While you have already undergone physical therapy, it may be worth exploring different modalities. Techniques such as transcutaneous electrical nerve stimulation (TENS), ultrasound therapy, or specialized exercises may provide relief.
4. Psychological Support: Engaging with a psychologist or counselor can help address any emotional or psychological factors contributing to your perception of pain. Cognitive-behavioral therapy (CBT) has been shown to be effective in managing chronic pain conditions.
5. Alternative Therapies: Some patients find relief through alternative therapies such as acupuncture, chiropractic care, or massage therapy. While the evidence for these treatments can vary, they may provide symptomatic relief for some individuals.
6. Surgical Options: If conservative treatments fail and the numbness significantly impacts your quality of life, revisiting the possibility of surgical intervention may be warranted. This could involve exploring the surgical site for any residual compression or addressing scar tissue.
Conclusion
Persistent genital numbness after disc surgery can be a challenging condition to manage. It is essential to work closely with a multidisciplinary team of healthcare providers, including neurologists, pain specialists, and physical therapists, to develop a comprehensive treatment plan tailored to your specific needs. While recovery may take time, many patients do experience improvement with appropriate interventions. It is crucial to remain proactive in seeking solutions and to communicate openly with your healthcare team about your symptoms and concerns.
