The feasibility of extracting upper molars for orthodontic treatment?
I would like to ask about my situation.
I have protruding teeth and I am considering orthodontic treatment to improve my condition.
After consulting several clinics, they all mentioned that I would need to extract teeth to create space for retraction.
Since I have already had my first upper premolar extracted due to it being completely impacted on the inside, I am facing difficulties regarding which tooth to extract.
I mainly want to inquire about the extraction of upper teeth:
1.
One dentist mentioned that extracting any upper molar could lead to adjacent teeth potentially tilting because they are closer to the maxillary sinus, which provides more stability.
I would like to know the validity of this statement.
2.
I would also like to ask if extracting wisdom teeth could lead to similar issues, and what the perspective is on extracting wisdom teeth to create space for retraction.
Thank you very much.

Hello,
Due to the necessity of various clinical examinations for space analysis during orthodontic treatment, including intraoral examinations, impressions, full-mouth X-rays, and cephalometric analysis, it is essential to establish the most suitable treatment plan for the patient.
This is particularly important when considering extractions, as a thorough intraoral assessment is required to make an informed decision.
When it comes to the posterior teeth in orthodontics, there is usually a preference for moving the teeth forward, making the extraction of wisdom teeth less common for creating space.
If you have any further orthodontic concerns, we recommend consulting with our clinic or another orthodontic specialist.
Thank you for your inquiry.
Wishing you peace and good health.
Sincerely,
Dr.

When considering orthodontic treatment for conditions such as overbite (often referred to as "bulldog teeth"), extraction of teeth can be a common recommendation to create space for proper alignment. Your inquiry about the feasibility of extracting upper molars for orthodontics raises several important points that warrant discussion.
1. Extraction of Upper Molars and Tooth Movement: The concern raised by your dentist regarding the extraction of upper molars and the potential for adjacent teeth to shift improperly is valid. The upper molars are located near the maxillary sinus, and their extraction can indeed affect the stability of neighboring teeth during orthodontic movement. When a tooth is extracted, the surrounding teeth may experience a tendency to drift into the space left behind. This is particularly true for molars, which are larger and have more significant roots anchoring them to the jawbone. If the extraction site is not managed properly, it can lead to tilting or drifting of adjacent teeth, which could complicate the orthodontic treatment and potentially lead to misalignment.
2. Implications of Wisdom Tooth Extraction: Regarding the extraction of wisdom teeth (third molars), this procedure can sometimes create additional space in the dental arch, but it is essential to understand that wisdom teeth are often not directly involved in the alignment of the anterior teeth. Their removal may not significantly impact the positioning of the upper molars or the anterior teeth unless they are impacted or causing crowding. However, if wisdom teeth are extracted, it is crucial to monitor the movement of the remaining teeth closely to ensure they do not shift in undesirable ways.
3. Orthodontic Treatment Planning: The decision to extract teeth, whether molars or premolars, should be made based on a comprehensive orthodontic evaluation. This includes assessing the overall dental arch, the position of the teeth, the relationship between the upper and lower jaws, and the specific goals of treatment. An orthodontist will typically use diagnostic tools such as X-rays, photographs, and dental impressions to create a treatment plan that minimizes the risk of complications.
4. Alternative Options: In some cases, orthodontists may explore alternative methods to create space without extractions. These can include the use of expanders to widen the dental arch or other techniques that may help in aligning the teeth without the need for extractions. However, the feasibility of these options largely depends on the individual’s dental anatomy and the severity of the malocclusion.
5. Post-Extraction Care: If extractions are deemed necessary, proper post-operative care is crucial. This includes following the orthodontist's instructions regarding oral hygiene, diet, and any prescribed medications to ensure a smooth recovery and to facilitate the subsequent orthodontic treatment.
In conclusion, while extracting upper molars can be a viable option for creating space in orthodontic treatment, it is essential to consider the potential implications on adjacent teeth and the overall treatment plan. Consulting with an experienced orthodontist who can evaluate your specific situation and provide tailored recommendations is crucial. They will be able to guide you through the process, ensuring that you achieve the best possible outcome for your dental health and aesthetics.
