Root canal treatment

Hello doctor, I would like to ask about an issue I experienced.
I had a root canal treatment at the dental clinic yesterday, but this afternoon I started experiencing severe throbbing pain.
I have been taking the medication prescribed by the dentist on schedule, but the pain remains intense.
What could be the possible reasons for this?

Enzo, 20~29 year old female. Ask Date: 2017/03/21

Experiencing severe pain after a root canal treatment can be distressing and confusing. There are several potential causes for this pain, and understanding them can help you manage your symptoms and communicate effectively with your dentist.

1. Incomplete Cleaning of the Root Canals: One of the most common reasons for pain after a root canal is that not all of the infected or inflamed pulp tissue was removed. The root canal system can be complex, with multiple canals and branches that may not be visible on X-rays. If any infected tissue remains, it can lead to persistent pain and inflammation.

2. Pressure Build-Up: During the procedure, if the root canal is not adequately sealed or if there is inflammation in the surrounding tissues, pressure can build up within the tooth. This pressure can cause significant pain, especially when biting down or applying pressure to the tooth.

3. Post-Operative Inflammation: After a root canal, it is normal for some inflammation to occur as the body responds to the treatment. This inflammation can cause discomfort and pain, which may feel like a throbbing or sharp sensation.

4. Infection: If bacteria were not completely eliminated during the procedure, or if new bacteria entered the canal after treatment, an infection can develop. This can lead to severe pain and may require additional treatment, such as antibiotics or further root canal therapy.

5. Nerve Damage: Although the nerve inside the tooth is typically removed during a root canal, the surrounding tissues and nerves can still be affected. If there is trauma to these areas during the procedure, it can result in pain that may not respond well to standard pain medications.

6. Jaw and Muscle Strain: Sometimes, the position of your jaw during the procedure can lead to muscle strain, which can cause discomfort and pain in the days following the treatment.

Solutions and Recommendations:
- Follow-Up with Your Dentist: It is crucial to communicate with your dentist about the pain you are experiencing. They may want to examine the tooth again, possibly taking new X-rays to assess the situation.

- Pain Management: Continue taking any prescribed pain medications as directed. Over-the-counter pain relievers like ibuprofen can also be effective in managing pain and reducing inflammation.

- Cold Compress: Applying a cold compress to the outside of your cheek can help reduce swelling and numb the area, providing temporary relief from pain.

- Avoid Hard Foods: Stick to soft foods and avoid chewing on the affected side until the pain subsides and your dentist has evaluated the situation.

- Antibiotics: If there is a suspicion of infection, your dentist may prescribe antibiotics to help control the infection and reduce pain.

- Consider Further Treatment: If the pain persists despite treatment, your dentist may recommend retreatment of the root canal or referral to an endodontist, a specialist in root canal procedures.

In conclusion, while experiencing pain after a root canal can be concerning, it is essential to remember that it can be a normal part of the healing process. However, if the pain is severe and persistent, it is crucial to seek further evaluation and treatment from your dentist to address any underlying issues effectively.
