Postoperative symptoms after fistula surgery?

Hello, I underwent a fistula repair surgery at a hospital near Madou on February 14th due to a fever.
It has been over a month since the surgery.

1.
Although the surgical wound has healed, I occasionally experience swelling and pain beneath the wound, especially at night, requiring me to take pain medication.

2.
After using the restroom, I often experience intense itching and stinging around the anus, and sometimes there is a small amount of bleeding, even if I don't touch the area.
The itching is so severe that it feels unbearable.
I have returned to the clinic two or three times, and the doctors have told me that this is normal.
However, this level of itching is intolerable, so I bought Betadine ointment from the pharmacy to apply myself.

3.
The doctor advised me not to take sitz baths after the first week post-surgery.
During follow-up visits, the doctors have instructed me not to wipe too thoroughly after using the restroom and not to wash the anal area with water.
They also asked me to insert a finger into the anus to help with bowel movements.

Could you please tell me if my symptoms are considered normal? It seems that during my follow-up visits, the doctors do not pay much attention to these issues and are primarily focused on my bowel training.
As a police officer with irregular working hours, the pain, itching, and discomfort in the groin area have significantly affected my daily life.

More Info

After undergoing fistula surgery, it is common to experience a range of post-operative symptoms, some of which you have described. It's important to understand that while certain symptoms can be considered normal, others may require further evaluation and management.
1. Pain and Swelling: It is not unusual to experience pain and swelling around the surgical site for several weeks after surgery. The healing process can cause discomfort, especially if the area is irritated or if there is any movement that pulls on the healing tissue. If you find that the pain is manageable with over-the-counter pain medications, this is generally a good sign. However, if the pain worsens or becomes unmanageable, it is crucial to consult your healthcare provider.

2. Itching and Discomfort: Itching and a sensation of discomfort around the anal area can occur after fistula surgery. This can be due to several factors, including healing tissue, irritation from bowel movements, or even the use of toilet paper. It is important to avoid excessive wiping, as this can exacerbate irritation. Instead, consider using moist wipes or a gentle cleansing method after using the restroom. If you are experiencing significant itching and discomfort, topical treatments like hydrocortisone cream or other soothing ointments may provide relief, but it is best to discuss this with your doctor before self-treating.

3. Bleeding: Occasional minor bleeding can occur, especially if there is irritation or if the area is not fully healed. However, if you notice persistent or heavy bleeding, it is essential to seek medical attention. Your doctor may want to assess the surgical site to ensure there are no complications, such as infection or improper healing.

4. Post-operative Care: Your doctor's advice to avoid sitting baths and to be gentle with the area is standard post-operative care. It is essential to follow these guidelines to promote healing and prevent complications. If you feel that your concerns are not being adequately addressed during follow-up visits, consider seeking a second opinion or discussing your symptoms more assertively with your healthcare provider.

5. Lifestyle Adjustments: Given your work schedule and the physical demands of your job, it may be beneficial to explore ways to reduce strain on your body. This could include taking breaks to rest, adjusting your sitting position, and ensuring you are not putting undue pressure on the surgical site.
6. Psychological Impact: The discomfort and lifestyle changes following surgery can lead to frustration and emotional distress. It is important to acknowledge these feelings and consider discussing them with a healthcare professional. They may provide resources or referrals to support groups or counseling services that can help you cope with the changes you are experiencing.

In summary, while some of the symptoms you are experiencing after fistula surgery can be considered normal, it is crucial to monitor their severity and frequency. If your symptoms persist or worsen, do not hesitate to reach out to your healthcare provider for further evaluation and management. Your comfort and recovery are paramount, and addressing these issues can significantly improve your quality of life post-surgery.
