Abnormal bleeding
Hello doctor, I took a pregnancy test on March 22 (which was negative) and got my period on March 23.
Today, April 1, I had sexual intercourse, and three hours later, when I went to the bathroom, I noticed some bleeding when I wiped with toilet paper, and there was also a little bit of red blood on my underwear.
Could this indicate a possible pregnancy, or is it likely a menstrual irregularity? Thank you.
Vivian, 10~19 year old female. Ask Date: 2017/04/02
Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the factors affecting irregular bleeding outside of menstruation are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraception, diet, uterine or cervical lesions, infections, pregnancy, and breastfeeding, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you safety and health, Dr.
Zhang.

More Info
Abnormal bleeding can be a source of concern for many women, and understanding its potential causes is essential for addressing any underlying issues. Based on your description, it seems you experienced a normal menstrual cycle starting on March 23, followed by a sexual encounter on April 1, after which you noticed some bleeding. Let's break down the possible explanations for this bleeding.
1. Timing of the Bleeding: Since your last menstrual period began on March 23, the bleeding you observed on April 1 is quite close to the end of your menstrual cycle. If your cycles are typically around 28 days, you would be nearing the end of your cycle, which could explain some irregular spotting.
2. Ovulation and Implantation Bleeding: If you had unprotected intercourse, there is a possibility of ovulation bleeding or even implantation bleeding if conception occurred. Ovulation bleeding typically occurs mid-cycle, around 10 to 16 days after your last period, and is usually light and short-lived. Implantation bleeding, which can occur when a fertilized egg attaches to the uterine lining, may happen around a week after conception and can be mistaken for a light period. However, since you took a pregnancy test on March 22 and it was negative, it is less likely that you are pregnant unless the timing of ovulation was miscalculated.
3. Hormonal Fluctuations: Hormonal imbalances can lead to abnormal bleeding. Stress, changes in routine, or significant lifestyle changes can affect your hormonal levels, leading to unexpected bleeding. If you have been under stress or have had changes in your sleep patterns or diet, these could contribute to the irregular bleeding you are experiencing.
4. Physical Trauma or Irritation: Sometimes, bleeding can occur due to physical irritation or trauma during intercourse. If there was any friction or if you were not adequately lubricated, this could result in minor bleeding. This type of bleeding is usually not a cause for concern and should resolve on its own.
5. Infections or Other Conditions: Infections, such as sexually transmitted infections (STIs) or pelvic inflammatory disease (PID), can also cause abnormal bleeding. If the bleeding persists or is accompanied by other symptoms like pain, unusual discharge, or fever, it would be advisable to seek medical attention.
6. Next Steps: Given that you have already taken a pregnancy test, and it was negative, it may be beneficial to monitor the bleeding. If it continues or worsens, or if you experience any additional symptoms, it would be wise to consult a healthcare provider for further evaluation. They may recommend a pelvic exam, ultrasound, or blood tests to rule out any underlying conditions.
In summary, while the bleeding you experienced could be due to a variety of factors, including hormonal fluctuations, physical irritation, or even the timing of your menstrual cycle, it is essential to pay attention to your body and seek medical advice if you have concerns. Regular check-ups with a healthcare provider can help ensure that any potential issues are addressed promptly.
