Side effects of Livol (Liver Protectant)?

My father (who is 82 years old) has benign prostatic hyperplasia and the doctor prescribed him Alfuzosin and Tamsulosin, one tablet each per day.
He has been taking Tamsulosin for several years, and he just started taking Alfuzosin this year, having been on it for three months.
Recently, my father has been complaining of symptoms such as drowsiness, back pain, fatigue, and a runny nose, and he suspects these might be side effects of Alfuzosin.
I would like to ask, if a medication is going to have side effects, wouldn't they typically occur soon after starting the medication? My father is considering taking the medication every other day instead; is that advisable? Thank you.

Ms. Chen, 40~49 year old female. Ask Date: 2017/04/02

Dr. Xu Wencang reply Urology

For urinary symptoms, you may return for a follow-up appointment to discuss with your physician.

Alfuzosin, marketed under the brand name Uroxatral, is an alpha-1 adrenergic blocker commonly prescribed for the treatment of benign prostatic hyperplasia (BPH), particularly in elderly patients. While it is generally well-tolerated, it is important to understand its potential side effects, especially in older adults who may have multiple comorbidities and be taking various medications.

In your father's case, he has been taking alfuzosin (活路利淨) for three months, and he is experiencing symptoms such as drowsiness, back pain, fatigue, and nasal congestion. It is indeed possible for side effects to manifest after a period of use, not just at the initiation of treatment. This delayed onset can occur due to cumulative effects of the medication, interactions with other drugs, or changes in the patient's health status over time.

Common side effects of alfuzosin include dizziness, fatigue, headache, and gastrointestinal disturbances. However, in elderly patients, the risk of experiencing side effects may be heightened due to factors such as decreased renal function, polypharmacy (the use of multiple medications), and increased sensitivity to medications. Drowsiness and fatigue can be particularly concerning, as they may increase the risk of falls, which is a significant issue in older adults.

Regarding your father's symptoms, it is essential to consider that while alfuzosin may contribute to drowsiness and fatigue, other factors could also be at play. For instance, back pain could be related to musculoskeletal issues rather than a direct side effect of the medication. Nasal congestion might be due to environmental factors or other underlying conditions, such as allergies or a respiratory infection.

As for the idea of changing the dosing regimen to every other day, it is crucial to consult with your father's healthcare provider before making any adjustments. Alfuzosin is typically prescribed to be taken daily to maintain stable drug levels in the body for optimal efficacy. Altering the dosing schedule without medical guidance could lead to suboptimal treatment of BPH and may not alleviate the side effects.
If your father is experiencing significant discomfort or if the side effects are impacting his quality of life, it is advisable to discuss these concerns with his physician. The doctor may consider adjusting the dose, switching to a different medication, or implementing additional strategies to manage the side effects.
In summary, while alfuzosin is generally safe, elderly patients may experience side effects that can emerge after prolonged use. It is essential to monitor these symptoms closely and maintain open communication with healthcare providers to ensure safe and effective management of BPH and any associated side effects. Always prioritize professional medical advice when considering changes to medication regimens.
