Prepuce and glans issues?
I previously had an itchy condition that caused a tear in my foreskin, and after seeing a doctor and applying medication, it gradually improved.
However, there has always been a small tear, and after a few days, a small growth appeared next to it.
When pressed, it bleeds, and there is also a growth on the corresponding area of the glans.
At that time, I noticed numerous red spots, and it was painful when pressed.
Throughout this process, I have been applying medication, but I feel that the symptoms are getting worse.
The red spot on the foreskin has not disappeared, and the glans hurts when it comes into contact with water.
There is also some clear discharge, and the surrounding skin has started to become red, swollen, and inflamed.
Doctor, could you please explain why this is happening?

Hello: Please keep the area clean and dry, and avoid using soap for the time being.
Remember to dry it thoroughly if it gets wet.
If there is no improvement, please return to the urology department for a follow-up.
If you have engaged in unsafe sexual practices, please return immediately and inform the physician about this situation.
Thank you for your inquiry.

Understanding issues related to the prepuce (foreskin) and glans (the head of the penis) is crucial for maintaining male genital health. The symptoms you describe, including itching, pain, redness, and discharge, suggest a possible infection or inflammatory condition. Let's explore the potential causes, treatments, and preventive measures for these issues.
Causes
1. Infections: The most common causes of symptoms like yours are infections, which can be bacterial, viral, or fungal. For instance, balanitis is an inflammation of the glans, often caused by poor hygiene, irritants, or infections. Yeast infections, particularly from Candida species, can also lead to similar symptoms, especially if there is a predisposing factor like diabetes or antibiotic use.
2. Dermatological Conditions: Conditions such as eczema or psoriasis can affect the genital area, leading to redness, itching, and discomfort. These conditions may require specific dermatological treatments.
3. Sexually Transmitted Infections (STIs): Certain STIs, such as herpes or gonorrhea, can cause similar symptoms, including pain, discharge, and lesions. If there is a possibility of exposure to STIs, it is essential to get tested.
4. Allergic Reactions: Irritation from soaps, lotions, or condoms can lead to allergic reactions, resulting in redness and discomfort.
5. Phimosis: This condition occurs when the foreskin cannot be retracted fully over the glans, leading to inflammation and pain. It can be a contributing factor to recurrent infections.
Treatments
1. Topical Medications: Depending on the diagnosis, topical antifungal creams, antibacterial ointments, or corticosteroids may be prescribed to reduce inflammation and treat infections.
2. Oral Medications: If a bacterial infection is suspected, oral antibiotics may be necessary. For fungal infections, antifungal medications may be prescribed.
3. Hygiene Practices: Maintaining proper hygiene is crucial. Gently cleaning the area with mild soap and water can help prevent infections. It is essential to dry the area thoroughly after washing.
4. Avoiding Irritants: Identifying and avoiding products that cause irritation, such as scented soaps or lotions, can help alleviate symptoms.
5. Medical Procedures: In cases of phimosis or recurrent infections, a healthcare provider may recommend circumcision or other surgical interventions.
Preventive Measures
1. Regular Hygiene: Regular washing of the genital area is vital. For uncircumcised men, retracting the foreskin during washing is important to prevent the buildup of smegma, which can lead to infections.
2. Safe Sexual Practices: Using condoms can help reduce the risk of STIs. It is also important to communicate with sexual partners about health and any symptoms.
3. Monitoring Symptoms: Keeping an eye on any changes in symptoms and seeking medical advice promptly can prevent complications.
4. Healthy Lifestyle: Maintaining a healthy lifestyle, including a balanced diet and regular exercise, can support the immune system and overall health.
Conclusion
The symptoms you are experiencing are concerning and warrant further evaluation by a healthcare professional. It is crucial to address any underlying conditions and receive appropriate treatment. If symptoms persist or worsen, do not hesitate to seek medical attention. Early intervention can lead to better outcomes and prevent complications. Remember, maintaining good hygiene and being aware of any changes in your body are key components of genital health.
