After having wisdom teeth removed, the bite may become misaligned?
Hello Dr.
Huang, I had my upper and lower left wisdom teeth extracted on April 6th (the lower one was horizontally impacted).
Today, April 11th, I noticed in the mirror that my lower teeth have shifted entirely to the right.
The gap between my upper front teeth was previously aligned with the gap between the two middle lower teeth, but now the lower teeth have shifted to the right and are no longer aligned with the gap in the upper front teeth.
I have previously undergone orthodontic treatment and have consistently worn my retainers, so I am certain that my occlusion has always been correct.
Is this situation normal? If it is not normal, should I return to the dentist who extracted my wisdom teeth or to my orthodontist? Thank you, doctor.

Dear Ms.
Debby,
Hello! As mentioned, the lower teeth have shifted entirely to the right.
Generally, tooth extraction does not cause immediate movement of the teeth.
Please consult your orthodontist as soon as possible! Make sure to communicate with your doctor.
Thank you! Wishing you safety!
Sincerely,
Dr.

After wisdom tooth extraction, it is not uncommon for patients to experience some degree of dental misalignment, especially if the extraction involved significant manipulation of the surrounding teeth and bone. In your case, it appears that the lower teeth have shifted to the right, causing a misalignment with the upper front teeth. This situation can be concerning, particularly for someone who has previously undergone orthodontic treatment and has maintained their retainer.
Firstly, it is important to understand that some degree of shifting can occur after tooth extractions. The extraction site can create a space that may lead to adjacent teeth moving into that space, especially if the teeth were already in a precarious position. This is particularly true for wisdom teeth, which are often impacted and can exert pressure on neighboring teeth. However, significant misalignment, such as what you are describing, is not typical and may warrant further investigation.
Here are some steps you can take:
1. Monitor the Situation: In the initial weeks following the extraction, some shifting can occur as the surrounding tissues heal. It is advisable to monitor your dental alignment over the next few weeks. If the misalignment persists or worsens, then further action may be necessary.
2. Consult Your Dentist: Since you are experiencing a noticeable shift in your teeth, it would be prudent to return to the dentist who performed the extraction. They can assess the extraction site, check for any complications such as bone loss or infection, and determine if the misalignment is a result of the extraction process. They may also take X-rays to evaluate the position of the teeth and the health of the surrounding bone.
3. Orthodontic Evaluation: If your dentist determines that the extraction was performed correctly and there are no complications, it may be beneficial to consult with your orthodontist. They can evaluate the current alignment of your teeth and determine if any orthodontic intervention is necessary to correct the misalignment. Given your history of orthodontic treatment, your orthodontist may have specific insights into how to best address the situation.
4. Retainer Use: If you have a retainer, it is important to continue wearing it as directed by your orthodontist. Retainers are designed to help maintain the position of your teeth after orthodontic treatment. However, if the misalignment is significant, wearing the retainer may not be effective in correcting the shift. Your orthodontist may recommend a new retainer or adjustments to your current one.
5. Consider Future Treatment: Depending on the assessment from both your dentist and orthodontist, you may need to consider further treatment options. This could include additional orthodontic work to realign your teeth or other dental procedures to address any underlying issues caused by the extraction.
In summary, while some shifting of teeth can occur after wisdom tooth extraction, significant misalignment is not typical and should be evaluated by a dental professional. It is advisable to consult with the dentist who performed the extraction first, as they will have the most insight into the procedure and any potential complications. If necessary, they can refer you to your orthodontist for further evaluation and treatment options. Remember, timely intervention can help prevent further misalignment and ensure your dental health is maintained.
