Dizziness and a sense of imbalance?
Hello doctor, sometimes when I stand up suddenly after sitting, my vision goes black and I feel dizzy, and I have trouble walking steadily.
If I stand for a long time and then move suddenly, I also experience this.
Closing my eyes and resting for a moment helps.
I used to have a higher level of physical activity, and during training, I would easily become short of breath.
Halfway through my training, I suddenly felt weakness in my legs, tilted to the left, and everything went black with dizziness.
Even after closing my eyes and resting, I still felt weak in my legs.
It wasn't until I sat down and rested with my eyes closed for a while that I felt better.
I also tend to get motion sickness.
My health check showed no signs of anemia.
Could this be due to low blood sugar or other reasons?

Dr. Jiang Junyi reply Neurology
Hello, there are many factors that can cause dizziness, such as anemia, arrhythmias, and autonomic nervous system dysfunction.
Since your symptoms occur upon sudden standing, it is important to rule out dehydration and hypotension, which can lead to insufficient cerebral circulation.
You may improve your symptoms by replenishing fluids and electrolytes.
I recommend discussing further evaluation and treatment with a neurologist.
Wishing you good health and peace.
Best regards, Taoyuan Medical Center.

Dizziness and imbalance can be concerning symptoms that may arise from various underlying causes. Based on your description, it seems that you experience episodes of dizziness, particularly when standing up quickly or after prolonged standing, as well as feelings of weakness and imbalance. These symptoms can be attributed to several potential factors, and understanding them can help guide appropriate management.
One common cause of dizziness upon standing is orthostatic hypotension, which occurs when there is a sudden drop in blood pressure as you change positions. This can lead to feelings of lightheadedness or faintness, especially if you stand up too quickly after sitting or lying down. This condition can be exacerbated by dehydration, prolonged bed rest, or certain medications. It’s important to ensure that you are well-hydrated and to rise slowly from sitting or lying positions to mitigate these symptoms.
Another potential cause of your symptoms could be related to blood sugar levels. Hypoglycemia, or low blood sugar, can lead to dizziness, weakness, and even fainting. This is particularly relevant if you have been engaging in intense physical training or if your eating patterns have changed significantly. It’s advisable to monitor your blood sugar levels, especially if you notice that your symptoms correlate with periods of fasting or intense exercise.
Additionally, your description of feeling weak and leaning to one side could suggest a vestibular issue, which relates to the inner ear and balance. Conditions such as benign paroxysmal positional vertigo (BPPV) or vestibular neuritis can cause dizziness and imbalance. These conditions often require specific maneuvers or therapies to help alleviate symptoms.
Given your history of feeling faint during exercise and the episodes of weakness, it may also be worth considering cardiovascular factors. Conditions such as arrhythmias or other heart-related issues can lead to inadequate blood flow to the brain during exertion, resulting in dizziness and fainting. A thorough evaluation by a healthcare provider, including a physical examination and possibly an electrocardiogram (ECG), may be warranted to rule out any cardiovascular concerns.
It’s also important to consider other factors such as anxiety or panic attacks, which can manifest as dizziness and feelings of imbalance. Stress and anxiety can lead to hyperventilation, which may cause lightheadedness and a sense of disorientation.
In summary, your symptoms of dizziness and imbalance could stem from a variety of causes, including orthostatic hypotension, hypoglycemia, vestibular disorders, cardiovascular issues, or even anxiety. It is crucial to consult with a healthcare professional who can conduct a comprehensive evaluation, including a detailed medical history, physical examination, and possibly further testing, to determine the underlying cause of your symptoms. They can then recommend appropriate interventions or lifestyle modifications to help manage your condition effectively. In the meantime, consider keeping a symptom diary to track when your symptoms occur, what you were doing at the time, and any other relevant factors, as this information can be invaluable for your healthcare provider in making a diagnosis.
