Sudden Decrease in Menstrual Flow: Possible Causes and Concerns - Obstetrics and Gynecology

Menstrual flow suddenly decreases significantly?

Hello, Doctor.
I underwent a hysteroscopy on March 30 and a hysterosalpingography on March 31, which confirmed that there were no new polyps in the uterus and that my fallopian tubes are patent.
However, during this menstrual period, I have been experiencing significant abdominal pain, resembling bloating, along with a fever.
Additionally, my menstrual flow has been remarkably light.
I have adenomyosis, and my menstrual flow has always been heavy, but now, on the fourth day, aside from some flow yesterday, it has been so light that I don't even need to change my sanitary pad.
The color of the menstrual blood is also quite dark.
I would like to ask what might be the issue.
Thank you, Doctor!

Hello, I apologize for not being able to provide a diagnosis based on your description.
Bloating pain accompanied by fever could indicate inflammation.
If you continue to experience discomfort, it is advisable to consult a gynecologist.
Wishing you good health.

Sudden changes in menstrual flow can be concerning, especially when accompanied by other symptoms such as abdominal pain and fever. Based on your description, it seems you are experiencing a significant decrease in menstrual flow, which is unusual for you given your history of adenomyosis, a condition that often leads to heavier menstrual bleeding.
There are several potential causes for a sudden decrease in menstrual flow, particularly in the context of your recent medical procedures. Here are some possibilities:
1. Hormonal Changes: The menstrual cycle is regulated by hormones, and any disruption in hormonal balance can lead to changes in menstrual flow. The stress of undergoing medical procedures, such as a hysteroscopy and hysterosalpingography, can also affect hormone levels and subsequently impact your menstrual cycle.

2. Infection or Inflammation: The presence of fever and abdominal pain raises the possibility of an infection, such as endometritis, which is an infection of the uterine lining. This condition can occur after procedures involving the uterus and may lead to changes in menstrual flow, including a decrease in bleeding.

3. Uterine Scarring or Adhesions: If there was any trauma to the uterine lining during the procedures, it could lead to scarring or adhesions, which might affect menstrual flow. This is sometimes referred to as Asherman's syndrome, where scar tissue forms in the uterus, leading to lighter periods or even amenorrhea (absence of menstruation).

4. Pregnancy: Although it may seem unlikely given your history, any significant change in menstrual flow should prompt consideration of the possibility of pregnancy, especially if there is any chance of conception. A pregnancy test would be advisable if there is any doubt.

5. Adenomyosis Flare-Up: While adenomyosis typically causes heavy bleeding, it can also lead to irregularities in menstrual flow. The pain you are experiencing could be related to this condition, and it may be worth discussing with your healthcare provider.

Given your symptoms of abdominal pain, fever, and the drastic change in menstrual flow, it is crucial to seek medical attention promptly. A healthcare provider may recommend a pelvic examination, ultrasound, or other imaging studies to assess for any complications such as infection or structural changes in the uterus.
In summary, while there are several potential explanations for your symptoms, the combination of fever, abdominal pain, and decreased menstrual flow warrants further evaluation. It is essential to address these symptoms with your healthcare provider to ensure appropriate diagnosis and treatment. Please do not hesitate to reach out for medical assistance, as timely intervention can be vital in managing any underlying issues effectively.
