Hip joint swelling

Hello, Dr.
Shi.
I had a right hip joint replacement in 2005 due to a car accident, and in February 2015, I underwent another replacement because the ceramic liner and ceramic femoral head fractured.
Currently, in April 2017, I am experiencing swelling at the surgical site for unknown reasons.
Is this related to the artificial joint? Also, my surgeon is currently unavailable due to personal matters; can I wait until he returns to consult with him?

Benny, 40~49 year old female. Ask Date: 2017/04/19

Dr. Shi Guozheng reply Orthopedics

It is advisable to see a physician as soon as possible, as it may be related.

After undergoing hip joint replacement surgery, it is not uncommon for patients to experience swelling around the surgical site, even years after the procedure. In your case, having had two surgeries on the same hip joint, it is essential to consider several factors that could contribute to the swelling you are experiencing.

Firstly, the swelling could be a result of several potential causes, including but not limited to:
1. Post-Surgical Inflammation: After any surgical procedure, inflammation is a natural part of the healing process. This inflammation can persist for an extended period, especially if there have been complications or if the body is still adjusting to the artificial joint.

2. Infection: One of the most serious complications following joint replacement surgery is infection. Symptoms of infection may include increased swelling, redness, warmth, and pain at the surgical site. If you notice any of these symptoms, it is crucial to seek medical attention promptly.

3. Joint Effusion: This refers to the accumulation of excess fluid in the joint space. It can occur due to various reasons, including overuse, injury, or underlying conditions such as arthritis. Joint effusion can lead to swelling and discomfort.

4. Mechanical Issues: Given your history of a ceramic liner and femoral head fracture, it is possible that there could be mechanical issues with the current prosthesis. This could include loosening of the implant or wear and tear of the components, which might lead to inflammation and swelling.

5. Lymphatic Issues: Sometimes, swelling can occur due to lymphatic drainage issues, especially after surgeries that involve significant tissue manipulation.

Regarding your question about whether you can wait to see your surgeon until he returns, it is generally advisable to consult with a healthcare professional sooner rather than later, especially if the swelling is significant or accompanied by other concerning symptoms. While it may not be an emergency, it is essential to address any potential complications early to prevent further issues.

In the meantime, you can manage mild swelling with conservative measures such as:
- Rest: Avoid putting too much weight on the affected leg.

- Ice: Applying ice packs to the swollen area can help reduce inflammation.

- Elevation: Keeping the leg elevated can also assist in reducing swelling.

- Compression: Using a compression bandage may help manage swelling.

If the swelling persists or worsens, or if you experience increased pain, fever, or any other unusual symptoms, it is crucial to seek medical attention immediately. Your health and well-being should always be the priority, and timely intervention can make a significant difference in your recovery and overall quality of life.

In conclusion, while swelling after hip replacement surgery can be a normal part of the healing process, it is essential to monitor the situation closely and consult with a healthcare provider if you have any concerns. Your history of previous surgeries and the current symptoms warrant a thorough evaluation to ensure that there are no underlying complications that need to be addressed.
