Severe pain in the lower right side of the chest while breathing or speaking?

Hello doctor, I have noticed that for the past few days, I have been experiencing pain in the lower right side of my chest.
The pain occurs even when I breathe or talk, and it also radiates to my right shoulder.
It hurts so much that I can't tolerate it, whether I'm lying down, sitting, or standing, and even with minimal exertion.
I have researched my symptoms, but I'm still unsure about what might be happening.
I'm very scared that there could be something seriously wrong with my health.
Please help me.

Dr. Jiang Junyi reply Neurology

Hello, chest pain that radiates from the outside to the inside may initially suggest myofascial pain syndrome, which can present with pain, discomfort, and muscle stiffness.
However, pleuritis may also exhibit similar symptoms.
It is recommended that you consult a pulmonologist to discuss examination and treatment options.
Wishing you good health and peace.
Best regards, Taoyuan Medical Center.

Experiencing severe pain under the right chest during breathing and speaking can be concerning and warrants a thorough evaluation. There are several potential causes for this type of pain, ranging from musculoskeletal issues to more serious conditions involving the lungs, heart, or gastrointestinal tract.
One common cause of pain in the chest area, especially when it radiates to the shoulder, is musculoskeletal strain. This can occur due to activities that involve heavy lifting, awkward movements, or even prolonged poor posture. The muscles and ligaments in the chest and shoulder area can become strained, leading to pain that worsens with movement, deep breathing, or speaking. If you have recently engaged in any physical activity that could have led to such a strain, this might be a plausible explanation.

Another possibility is costochondritis, which is inflammation of the cartilage that connects a rib to the sternum. This condition can cause sharp pain in the chest that may worsen with certain movements or deep breaths. It is often benign but can be quite uncomfortable.

However, it is crucial to consider more serious conditions as well. For instance, pulmonary issues such as pneumonia, pleuritis (inflammation of the lining of the lungs), or even a pulmonary embolism (a blood clot in the lungs) can present with chest pain that worsens with breathing. If you are experiencing shortness of breath, coughing, or fever alongside your chest pain, these could be signs of a respiratory issue that requires immediate medical attention.

Cardiac issues should also be ruled out, especially if you have risk factors such as a history of heart disease, high blood pressure, or high cholesterol. Conditions like angina or even a heart attack can present with chest pain that may radiate to the shoulder, neck, or jaw. If you experience symptoms such as sweating, nausea, or lightheadedness, seek emergency medical care immediately.

Gastrointestinal issues, such as acid reflux or esophageal spasms, can also cause chest pain. If you notice that certain foods or eating habits exacerbate your symptoms, this might be a contributing factor.

Given the severity of your symptoms and the impact they have on your daily life, it is essential to consult a healthcare professional for a comprehensive evaluation. They may recommend imaging studies such as a chest X-ray or CT scan, blood tests, or even an electrocardiogram (ECG) to assess your heart function.
In the meantime, try to keep track of any additional symptoms you may experience, such as changes in your breathing pattern, any associated pain in other areas, or any triggers that worsen your condition. This information can be invaluable for your healthcare provider in determining the underlying cause of your pain.

In summary, while musculoskeletal issues are common and often benign, it is vital to rule out more serious conditions that could be causing your symptoms. Do not hesitate to seek medical attention, especially if your pain worsens or if you develop new symptoms. Your health and peace of mind are paramount, and a thorough evaluation will help you understand what is happening in your body and how to address it effectively.
