Penile issues

Hello Doctor, I have noticed that there are many small bumps in the coronal sulcus of my penis and on the inner side of the foreskin after an erection, which have been present for about ten years.
During my military physical examination, the doctor said they were papules and did not perform any further tests.
A few days ago, I visited a urologist out of concern, and the doctor only looked for a few seconds and said it was normal, attributing it to having a long foreskin and confirming it was not genital warts.
He also did not conduct any tests.
Can a doctor differentiate this just by visual inspection?

Zhi, 20~29 year old female. Ask Date: 2017/05/01

Dr. Du Shixi reply Urology

Hello: Visual inspection combined with inquiry can reveal about 90% of cases.
Unless there is suspicion that requires a definitive diagnosis through a biopsy for pathological examination, it is not necessary for everyone to undergo such procedures.

Pearly penile papules (PPP) are a common and benign condition that many men experience, particularly around the corona of the glans penis. These small, dome-shaped bumps are typically white or skin-colored and are often mistaken for other conditions, such as genital warts (caused by the human papillomavirus, or HPV). However, it's important to understand that PPP is not a sexually transmitted infection and does not require treatment unless the individual desires it for cosmetic reasons.

When you visited the urologist and they quickly assessed your condition, they likely relied on their clinical experience and visual examination to determine that what you were experiencing was indeed PPP. In most cases, trained healthcare providers can differentiate between PPP and other conditions based on appearance alone. PPP typically presents as multiple small, pearly, or flesh-colored papules that are arranged in a single row along the corona of the glans. They are harmless and do not cause any symptoms, such as itching or pain.

In contrast, genital warts usually appear as irregular, cauliflower-like growths and can be associated with discomfort or itching. If there is any uncertainty about the diagnosis, further testing, such as a biopsy, can be performed to confirm the nature of the lesions. However, this is rarely necessary for PPP, as the diagnosis is straightforward for most healthcare providers.

It's also worth noting that the presence of PPP is not related to hygiene or any underlying health issues. They are a normal anatomical variant and are particularly common in young men. If you have had these bumps for over ten years without any changes in size, shape, or symptoms, it is very likely that they are indeed pearly penile papules.

If you are still concerned about your condition or if you notice any changes in the appearance of the bumps, such as increased size, color changes, or the development of new lesions, it would be advisable to follow up with a healthcare provider. They can provide reassurance and, if necessary, discuss treatment options, which may include laser therapy or cryotherapy for those who wish to remove them for cosmetic reasons.

In summary, based on your description and the assessment by the urologist, it seems that what you are experiencing is likely pearly penile papules, which are benign and do not pose any health risks. However, if you have any lingering concerns or if your symptoms change, seeking further evaluation is always a good idea. Remember that open communication with your healthcare provider is key to addressing any health-related worries you may have.
