Right chest pain?

Hello, I used to experience sudden cramping sensations from my right chest to my right shoulder, which would worsen with deep breaths, but it wasn't frequent.
Recently, this same condition has occurred again and has become a bit more frequent (up to twice a day).
Additionally, I have been experiencing weakness in my right arm and sharp pain in my left chest (though the duration is short).
What could this be? Thank you for your response!

Dr. Zhou Ziguang reply Pulmonology

Hello: Based on what has been described, the cause is most likely neuromuscular pain, but please seek medical evaluation and treatment from a pulmonologist.

Understanding right chest pain can be quite complex, as it may stem from various causes, some of which are benign while others could be serious. Given your symptoms of right chest pain radiating to the shoulder, increased pain with deep breaths, and associated sensations of weakness in the right arm and stabbing pain in the left chest, it is essential to consider several potential causes.

1. Musculoskeletal Issues: One common cause of chest pain, especially if it is sharp and worsens with movement or deep breathing, is musculoskeletal strain. This can occur from activities that involve heavy lifting, poor posture, or even sleeping in an awkward position. Conditions like costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can also cause localized pain that may mimic more serious conditions.

2. Nerve Compression or Irritation: Another possibility is nerve-related pain, such as from a pinched nerve in the neck or upper back. This can lead to referred pain in the chest and shoulder area. If you have been experiencing any neck pain or discomfort, this could be a contributing factor.

3. Cardiac Concerns: While your symptoms may not immediately suggest a cardiac issue, it is crucial to rule out any heart-related problems, especially given the left-sided chest pain and arm weakness. Conditions such as angina or even a heart attack can present with atypical symptoms, particularly in younger individuals or those with risk factors. If you experience any additional symptoms such as shortness of breath, sweating, nausea, or prolonged pain, seek emergency medical attention immediately.

4. Pulmonary Issues: Conditions affecting the lungs, such as pleuritis (inflammation of the lining of the lungs) or pulmonary embolism (a blood clot in the lungs), can also cause sharp chest pain that worsens with breathing. If you have a history of blood clots, recent surgery, or prolonged immobility, these conditions should be considered.

5. Gastrointestinal Problems: Sometimes, gastrointestinal issues such as acid reflux or esophageal spasms can present as chest pain. If you notice that certain foods or positions exacerbate your symptoms, this could be a contributing factor.

Given the frequency of your symptoms and the presence of right arm weakness and left chest pain, it is advisable to seek medical evaluation. A healthcare professional may recommend diagnostic tests such as an electrocardiogram (ECG), chest X-ray, or even blood tests to assess for any underlying conditions.
In summary, while your symptoms may be related to musculoskeletal or nerve issues, the presence of left-sided chest pain and arm weakness warrants a thorough evaluation to rule out any serious conditions. It is always better to err on the side of caution when it comes to chest pain, so do not hesitate to seek medical attention if your symptoms persist or worsen.
