Postoperative issues related to hemorrhoid surgery?

Hello, Doctor.
On May 1st, I underwent hemorrhoid surgery and a self-funded coagulation procedure at Yadong Hospital.
Since then, whenever I sleep halfway, or when I stand or sit, my anal sphincter suddenly tightens, accompanied by severe pain that often makes me break out in a cold sweat! Is this situation normal? The pain is so intense that I am afraid to tighten my anus at all; it is very painful and quite distressing.
Additionally, during warm water sitz baths, I noticed a swollen lump near the anal area close to the vaginal opening, along with a small piece of tissue.
Is this a polyp? It hurts a lot when I touch it.
Thank you for your response, Doctor.

It is normal to experience pain at the surgical site within one week after hemorrhoidectomy.
It is recommended to avoid constipation and to take warm sitz baths.
If the pain persists and is not relieved, you may ask your physician for medication.
As for the polyp near the anus, it is likely due to the surgical site not being fully healed.
It is advisable to observe the situation for another two weeks post-surgery.

After undergoing hemorrhoid surgery, it is not uncommon for patients to experience a range of symptoms, including pain and discomfort. Your description of sudden, intense pain accompanied by muscle spasms in the anal sphincter is concerning and warrants further evaluation.
Firstly, it is important to understand that pain after hemorrhoid surgery can vary significantly among individuals. Some patients may experience mild discomfort, while others may have severe pain that can be exacerbated by certain activities such as sitting, standing, or even during bowel movements. The pain you describe, particularly the spasms of the anal sphincter, can be attributed to several factors:
1. Surgical Trauma: The surgical procedure itself can cause trauma to the surrounding tissues, leading to inflammation and pain. The healing process can also involve muscle spasms as the body reacts to the surgical intervention.

2. Nerve Sensitivity: After surgery, the nerves in the area may become hypersensitive, leading to increased pain perception. This can result in sudden, sharp pain that feels like spasms.

3. Postoperative Complications: In some cases, complications such as infection, abscess formation, or the development of scar tissue can lead to increased pain. It is essential to monitor for signs of infection, such as fever, increased redness, swelling, or discharge from the surgical site.

Regarding the lump you mentioned near the vaginal area, it is crucial to have this evaluated by your healthcare provider. While it could be a normal part of the healing process, such as a small hematoma or scar tissue, it could also indicate a more serious issue like a recurrence of hemorrhoids or a different type of growth.
Here are some recommendations for managing your symptoms and ensuring proper healing:
- Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain. If your pain is severe, your doctor may prescribe stronger medications.

- Warm Sitz Baths: Taking warm sitz baths can help soothe the area and reduce discomfort. This can also promote blood flow to the area, aiding in healing.

- Dietary Adjustments: To prevent straining during bowel movements, ensure you are consuming a high-fiber diet and staying well-hydrated. This can help soften stools and reduce the need for straining, which can exacerbate pain.

- Follow-Up Care: It is crucial to attend follow-up appointments with your surgeon or healthcare provider. They can assess your healing progress and address any complications that may arise.

- Avoiding Strain: Try to avoid activities that may put pressure on the surgical site, such as heavy lifting or prolonged sitting. If you experience pain during specific activities, it may be best to modify those activities until you have healed more fully.

In conclusion, while some pain and discomfort after hemorrhoid surgery are expected, the severity and nature of your symptoms should be evaluated by a healthcare professional. It is essential to communicate openly with your doctor about your pain levels and any new symptoms you experience. They can provide tailored advice and treatment options to help you manage your recovery effectively. If your symptoms persist or worsen, do not hesitate to seek medical attention promptly.
