Rectal bleeding

Hello Doctor, three days ago I experienced severe pain and bleeding during a bowel movement.
After wiping twice, the bleeding stopped.
I have a regular bowel movement every day, and I have had similar experiences in the past where it resolved the next day.
Over the past three days, I have had normal bowel movements without any further bleeding.
However, this time I feel a sharp pain whenever I exert pressure on my anus or sit down, and this has persisted for three days.
I would like to know if I need medication or if I should see a doctor.
Thank you.

Based on your description, it seems more like the typical symptoms of an anal fissure, which include pain during bowel movements accompanied by slight bleeding.
The common causes are often constipation or diarrhea.
The fundamental treatment is to improve bowel habits, and warm sitz baths can also be beneficial.
You can also purchase hemorrhoid suppositories at the pharmacy for relief.
If symptoms persist without improvement, it is advisable to consult a physician for further evaluation.

Rectal bleeding can be a concerning symptom, and understanding when to seek medical attention is crucial for proper diagnosis and treatment. Based on your description, it seems that you experienced significant pain and bleeding during bowel movements, which has since subsided, but you are still experiencing discomfort when straining or sitting.
Firstly, it's important to recognize that rectal bleeding can arise from various causes, some of which may be benign, while others could indicate a more serious condition. Common causes of rectal bleeding include hemorrhoids, anal fissures, diverticular disease, inflammatory bowel disease (IBD), and colorectal polyps or cancer. Given that you have experienced similar symptoms in the past that resolved quickly, it is possible that you may be dealing with a minor issue such as hemorrhoids or an anal fissure, both of which can cause pain and bleeding during bowel movements.

In your case, the fact that the bleeding has stopped and you are having normal bowel movements is a positive sign. However, the persistent pain when straining or sitting suggests that there may be an underlying issue that needs to be addressed. Here are some considerations:
1. Monitor Symptoms: Keep track of any changes in your symptoms. If the pain worsens, if you notice any new bleeding, or if you experience other symptoms such as fever, weight loss, or changes in bowel habits, it is essential to seek medical attention promptly.

2. Home Care: In the meantime, you may consider some home remedies to alleviate discomfort. This can include:
- Warm Sitz Baths: Soaking in warm water can help soothe the anal area and reduce pain.

- Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help manage pain.

- Dietary Adjustments: Ensure you are consuming a high-fiber diet to prevent constipation, which can exacerbate pain and lead to further straining during bowel movements.

3. When to Seek Medical Attention: You should consider seeing a healthcare provider if:
- The pain persists or worsens.

- You experience any new symptoms, such as fever or significant changes in bowel habits.

- You notice any additional bleeding, especially if it is bright red or accompanied by clots.

- You have a history of gastrointestinal issues or rectal bleeding that has not been evaluated.

4. Medical Evaluation: If you decide to see a doctor, they may perform a physical examination, including a digital rectal exam, and possibly recommend further tests such as a colonoscopy, especially if there is a concern for more serious conditions.

In summary, while your symptoms may resolve on their own, it is essential to remain vigilant. If you experience any worsening of symptoms or new concerns, do not hesitate to seek medical attention. Early evaluation can help rule out serious conditions and provide you with the appropriate treatment to ensure your health and well-being.
