Dizziness, body heat, discomfort, and dry heaving?

Hello, Doctor! Recently, I've been experiencing dizziness and a feeling of warmth in my body after walking or standing for about a minute.
My walking becomes unstable, especially during exercise, and I feel a pressure in my head along with sharp pain in the back of my head and tinnitus.
At times, I also experience chest pain.
I have undergone an electrocardiogram (ECG) and a stress test, both of which were normal.
I would like to know what symptoms I might be experiencing and which specialist I should consult.
Additionally, I previously took sublingual tablets due to chest pain, which has decreased recently, possibly because I have been exercising less.

An Zai, 20~29 year old female. Ask Date: 2017/06/06

Dear readers:
"I have been experiencing dizziness when walking or standing for about a minute." Here are some responses:
1.
Consider two aspects:
(1) Orthostatic hypotension: This occurs when standing up suddenly after prolonged sitting or lying down, leading to insufficient blood flow to the brain.
It may help to slow down your movements or avoid sudden standing.
(2) Dizziness: This may be caused by issues related to the vestibular system in the inner ear.
2.
It is advisable to consult a healthcare facility and seek further evaluation from a cardiologist or an otolaryngologist.
Thank you!

Dizziness and discomfort can be concerning symptoms, especially when they occur during activities like walking or standing. Based on your description, it seems you are experiencing a combination of dizziness, a sensation of heat, instability while walking, and even some headaches and ear ringing. These symptoms can be attributed to various underlying conditions, and it's essential to approach them with a comprehensive understanding.

Firstly, dizziness can stem from several causes, including vestibular disorders (related to the inner ear), cardiovascular issues, neurological conditions, or even anxiety. Given that you mentioned experiencing dizziness particularly during physical activity, it may be worthwhile to consider cardiovascular factors. Although your previous electrocardiograms (ECGs) and exercise stress tests returned normal results, it’s important to remember that these tests may not capture transient issues that could occur during exertion.

The sensation of heat and instability could indicate a drop in blood pressure, particularly orthostatic hypotension, which occurs when standing up quickly. This condition can lead to dizziness and is often exacerbated by dehydration or prolonged periods of inactivity. If you have been less active recently, this could contribute to your symptoms.
The headaches you described, particularly the throbbing sensation in the back of your head, could be tension-type headaches or even migraines, which can sometimes be triggered by physical exertion or stress. The presence of tinnitus (ear ringing) can also be associated with migraines or other vestibular disorders.

Moreover, the heart pain you mentioned, although less frequent now, should not be overlooked. Even if your heart tests were normal, it is crucial to discuss these symptoms with a healthcare provider, especially if they recur or worsen. Heart-related issues can sometimes present atypically, and further evaluation may be necessary.

Given the complexity of your symptoms, it would be prudent to seek medical advice from a healthcare professional who can perform a thorough evaluation. A primary care physician or a cardiologist would be appropriate starting points. They may recommend additional tests, such as blood pressure monitoring, blood tests to check for anemia or electrolyte imbalances, or even imaging studies if deemed necessary.

In summary, dizziness and discomfort can arise from various causes, and while some may be benign, others could indicate more serious underlying conditions. It is essential to take these symptoms seriously and consult with a healthcare provider who can guide you through the diagnostic process and help determine the best course of action. Regular follow-ups and open communication about your symptoms will be key in managing your health effectively.
