Varicose veins

Hello Doctor, I previously asked a question regarding my condition (#124595) and after going to the hospital, I was diagnosed with varicocele.
I would like to ask: 1.
Is there a high chance of recurrence after surgery? 2.
Should I avoid strenuous activities, such as playing basketball? 3.
I have done some research, but other sources online still say that masturbation can worsen the condition.
Does appropriate masturbation still exacerbate the pain? 4.
Previously, I mentioned a small, soft lump; could it be an epididymal cyst? Thank you for your responses!

Xiao Gao Er Sheng, 10~19 year old female. Ask Date: 2017/06/19

Dr. Du Shixi reply Urology

Hello: 1) The chance of recurrence is not small (especially if postoperative lifestyle habits are not changed).
2) Yes, vigorous exercise may lead to recurrence; prolonged sitting or standing is also not good, and severe coughing and heavy physical labor are detrimental as well.
3) Masturbation may have some impact, but it is very minimal.
4) It could be the epididymis, but the veins of a varicocele may also feel like small lumps.
Wishing you good health.

Varicocele is a condition characterized by the enlargement of veins within the scrotum, similar to varicose veins that can occur in the legs. This condition can lead to various complications, including infertility and discomfort. Understanding the risks, recovery, and lifestyle tips associated with varicocele is crucial for managing this condition effectively.

1. Recurrence After Surgery: The surgical treatment for varicocele, typically a varicocelectomy, has a relatively low recurrence rate. Studies indicate that the recurrence rate can range from 5% to 15%, depending on various factors, including the surgical technique used and the skill of the surgeon. While most patients experience significant relief from symptoms and improved fertility post-surgery, it is essential to follow up with your healthcare provider to monitor for any signs of recurrence.

2. Avoiding Strenuous Activities: After surgery, it is generally advisable to avoid strenuous activities, including high-impact sports like basketball, for a period of time. Engaging in such activities too soon can increase the risk of complications, such as bleeding or swelling. Most doctors recommend a recovery period of at least two weeks before gradually resuming normal activities, including exercise. However, it is crucial to listen to your body and consult your surgeon for personalized advice based on your recovery progress.

3. Masturbation and Pain: There is a common concern regarding whether masturbation can exacerbate pain associated with varicocele. While some individuals report discomfort during or after masturbation, this is not universally experienced. The key is moderation and paying attention to your body’s signals. If masturbation leads to increased pain or discomfort, it may be wise to limit the frequency or consult with a healthcare provider for tailored advice. Generally, moderate sexual activity, including masturbation, should not significantly worsen varicocele symptoms, but individual experiences may vary.

4. Understanding the Soft Lump: The small, soft lump you mentioned could potentially be the epididymis, which is a coiled tube located at the back of the testicle that stores and matures sperm. It is normal for the epididymis to feel soft and movable. However, if you have concerns about any lumps or changes in your scrotum, it is essential to have them evaluated by a healthcare professional to rule out any other conditions, including hernias or tumors.

Lifestyle Tips: Managing varicocele effectively also involves lifestyle modifications. Here are some tips:
- Maintain a Healthy Weight: Obesity can exacerbate varicocele symptoms. A balanced diet and regular exercise can help maintain a healthy weight.

- Avoid Prolonged Standing or Sitting: If your job requires long periods of standing or sitting, take breaks to move around. This can help improve blood circulation and reduce pressure in the veins.

- Wear Supportive Underwear: Supportive undergarments can help alleviate discomfort by providing support to the scrotum.

- Stay Hydrated and Eat a Balanced Diet: Proper hydration and nutrition can improve overall health and potentially reduce symptoms.

- Limit Heavy Lifting: If possible, avoid heavy lifting or straining, as this can increase pressure in the abdominal cavity and worsen varicocele symptoms.

In conclusion, while varicocele can pose challenges, understanding the risks, recovery process, and making informed lifestyle choices can significantly improve your quality of life. Always consult with your healthcare provider for personalized advice and follow-up care to ensure the best outcomes.
