Lower abdominal tenderness upon palpation?

Hello, doctor! I would like to ask if the pain in my lower left abdomen when pressed could be due to muscle inflammation? Because when I don't press on it, I hardly feel anything and there is no spontaneous abdominal pain.

Niming, 20~29 year old female. Ask Date: 2017/06/21

Hello: If there is no pain when not pressing, it is not recommended to apply pressure.
Pain may be discomfort caused by excessive force.

Left lower abdominal pain can be a source of concern for many individuals, and your description of pain that occurs primarily with pressure suggests a few potential causes. Based on your symptoms, it seems likely that the discomfort you are experiencing is related to muscle inflammation or strain rather than a more serious internal condition.

When pain is localized and only occurs upon palpation, it often indicates a musculoskeletal issue. Muscle inflammation, also known as myositis, can result from overuse, strain, or injury. If you've engaged in activities that require significant use of your abdominal muscles—such as heavy lifting, intense workouts, or even prolonged periods of poor posture—this could lead to muscle soreness or inflammation. The fact that you do not experience pain spontaneously or during normal activities further supports this possibility.

In contrast, pain associated with internal organs, such as the intestines, bladder, or reproductive organs, typically presents with additional symptoms. For example, conditions like appendicitis, diverticulitis, or urinary tract infections often come with systemic symptoms such as fever, nausea, changes in bowel habits, or urinary symptoms. Since you mentioned that the pain is not present unless you apply pressure, it is less likely to be related to these internal conditions.

It's also worth considering the possibility of referred pain. Sometimes, issues in one area of the body can manifest as pain in another. For instance, conditions affecting the lower back or hips can sometimes cause discomfort in the lower abdomen. However, if the pain is strictly localized to the left lower abdomen and only occurs with pressure, this is less likely.

If you suspect muscle inflammation, there are several self-care strategies you can employ. Resting the affected area, applying heat or cold packs, and taking over-the-counter anti-inflammatory medications (like ibuprofen) can help alleviate discomfort. Gentle stretching and strengthening exercises may also be beneficial, but it's essential to avoid activities that exacerbate the pain.

However, if the pain persists, worsens, or if you begin to experience other symptoms such as fever, significant changes in bowel habits, or urinary issues, it would be prudent to seek medical evaluation. A healthcare provider can perform a physical examination and may recommend imaging studies or lab tests to rule out any underlying conditions.

In summary, based on your description of the pain being present only upon pressure and not spontaneously, it is likely related to muscle inflammation or strain. Monitoring your symptoms and practicing self-care can be effective, but do not hesitate to consult a healthcare professional if your condition does not improve or if you have any concerns about your health.
