Postoperative care after retinal laser treatment?

Hello Doctor: Yesterday, I went to the ophthalmology department for a fundus examination and found a hole in the retina of my left eye.
Immediate laser treatment was performed, and the doctor advised me to return for a follow-up in three months.
Today, I sometimes notice a shadow in the lower visual field of my left eye, but it disappears when I shift my gaze.
I would like to ask if this is a normal phenomenon after the laser treatment.

Niming, 20~29 year old female. Ask Date: 2017/06/29

Dr. Gao Pilin reply Ophthalmology

Hello: Sometimes shadows are fragments of retinal tears, sometimes they are blood clots, and sometimes they are related to floaters.
It is still recommended to have an outpatient examination.
If the examination shows no issues, there is no need to worry.
In Taiwan, at Puzi Hospital's Ophthalmology Department: Monday to Friday from 8:00 AM to 12:00 PM, and Wednesday afternoon from 1:00 PM to 4:30 PM.

After undergoing laser treatment for a retinal hole, it's understandable to have concerns about your recovery and the symptoms you're experiencing. The presence of shadows or fluctuations in your vision, particularly after a recent procedure, can be common. However, it’s essential to monitor these symptoms closely and communicate with your healthcare provider.

When a retinal hole is treated with laser therapy, the goal is to create a scar around the hole to prevent fluid from passing through and causing a retinal detachment. Immediately after the procedure, it’s not unusual to experience some visual disturbances, including shadows or changes in your visual field. These symptoms can occur due to several reasons:
1. Healing Process: After laser treatment, the retina is in a healing phase. The laser creates a thermal effect that can lead to temporary swelling or changes in the retinal structure. This may manifest as shadows or distortions in your vision, particularly in the area surrounding the treated site.

2. Vitreous Changes: The vitreous gel that fills the eye can also change after laser treatment. If the vitreous is pulling away from the retina (a common occurrence with aging or after surgery), it can create visual phenomena such as flashes or floaters, and sometimes shadows.

3. Post-Procedure Sensitivity: Your eye may be more sensitive after the procedure, and this can lead to temporary visual disturbances. This sensitivity should gradually decrease as your eye heals.

4. Normal Variability: Vision can fluctuate for various reasons, including lighting conditions, fatigue, or even the position of your head. If the shadow you see disappears when you shift your gaze, it may not be a cause for concern.

However, while some visual disturbances can be normal post-laser treatment, it’s crucial to be vigilant. Here are some signs that warrant immediate medical attention:
- Persistent Shadows: If the shadows or visual disturbances do not improve or worsen over time.

- Flashes of Light: If you experience new flashes of light or an increase in floaters.

- Vision Loss: Any sudden loss of vision or significant changes in your visual acuity should be evaluated promptly.

In terms of recovery, it’s essential to follow your doctor's advice regarding follow-up appointments. The three-month follow-up is standard to assess the healing process and ensure that the laser treatment was effective. During this time, you should also maintain a healthy lifestyle, including a balanced diet rich in antioxidants (like leafy greens and fish) that support eye health.

Additionally, avoid activities that could strain your eyes, such as prolonged screen time without breaks, and protect your eyes from bright lights or UV exposure. If you have any concerns about your vision or the healing process, do not hesitate to reach out to your eye care provider for guidance.

In summary, while some visual disturbances can be expected after laser treatment for a retinal hole, it’s essential to monitor your symptoms closely and maintain open communication with your healthcare provider. Your vision is precious, and proactive management is key to ensuring the best possible outcome.
