Persistent Ear Swelling: Seeking Solutions for Unexplained Discomfort - Otolaryngology

Hearing is normal, but the constant feeling of swelling severely affects daily life?

Recently, I started to worry because I suddenly felt swelling in my ear, but it was intermittent at first.
However, during the summer vacation, it became persistent day and night.
Since I needed to prepare for exams, I felt it was significantly affecting me, so I went to see a doctor.
Initially, I visited an otolaryngologist, but after testing my ear pressure and finding it normal, the doctor said there was no medication to prescribe.
A few days later, I went to another clinic, and after a hearing test that also came back normal, the doctor could only speculate that it was due to allergies causing dysfunction of the Eustachian tube and prescribed me a nasal spray.
However, I haven't experienced any nasal discomfort, congestion, or runny nose recently, and after using the spray for a week, there has been no improvement.
I then consulted a traditional Chinese medicine practitioner, who said I had excessive heat and prescribed me some herbal medicine.
After taking it for several days, the swelling sensation persists, and I increasingly feel occasional mild pain in my eardrum, along with pain radiating from my temples to my neck, which feels like a continuous line.
I also find it easier to feel dizzy and have difficulty concentrating.
Doctor, I am currently unsure about what to do and am very worried about not being able to resolve this issue.
What should I do now?

Is your ear pressure normal? Is there anything abnormal in your nasopharynx? You may need to visit an otolaryngologist for an evaluation to get an answer.
Wishing you well.

Persistent ear swelling and discomfort can be quite distressing, especially when it affects your daily life and concentration during important times like exam preparation. Based on your description, it seems that you've already consulted multiple healthcare providers, including an ENT specialist and a traditional Chinese medicine practitioner, but have not yet found a satisfactory resolution to your symptoms.

First, it's important to understand that ear swelling can be caused by a variety of factors, including allergies, infections, fluid buildup, or even referred pain from other areas such as the jaw or neck. Given that your initial tests for ear pressure and hearing were normal, the suggestion that your symptoms might be related to Eustachian tube dysfunction due to allergies is plausible. However, since you mentioned that you do not have any nasal symptoms like congestion or runny nose, it might be worth exploring other potential causes.

The fact that you are experiencing pain that radiates from your ear to your temples and neck, along with dizziness and difficulty concentrating, raises the possibility of a few different conditions. One potential cause could be temporomandibular joint (TMJ) dysfunction, which can lead to referred pain in the ear and surrounding areas. This condition can be exacerbated by stress, teeth grinding, or jaw clenching, which may be more common during periods of anxiety, such as preparing for exams.

Another possibility is that you may be experiencing a form of otitis media, which is an infection or inflammation of the middle ear. Even if your hearing test was normal, there could still be fluid present that is not easily detected. This could lead to a sensation of fullness or swelling in the ear, as well as discomfort.

Given your ongoing symptoms and the lack of improvement with the treatments you've tried, I would recommend the following steps:
1. Follow-Up with an ENT Specialist: Since your symptoms are persistent, it may be beneficial to return to an ENT specialist for a more comprehensive evaluation. They may consider imaging studies, such as a CT scan, to rule out any structural issues or fluid accumulation that may not have been evident in previous examinations.

2. Consider TMJ Evaluation: If you suspect that your symptoms might be related to jaw issues, consulting with a dentist or an oral and maxillofacial specialist who is experienced in TMJ disorders could provide insights. They may recommend treatments such as a night guard to prevent teeth grinding or exercises to relieve jaw tension.

3. Explore Allergy Testing: If allergies are suspected, consider seeing an allergist for testing. Even if you do not have typical allergy symptoms, some individuals can experience ear-related symptoms due to environmental allergens.

4. Pain Management: For the pain you are experiencing, over-the-counter pain relievers such as ibuprofen or acetaminophen may help alleviate discomfort. However, consult with your healthcare provider before starting any new medication.

5. Stress Management Techniques: Since you are preparing for exams, incorporating stress management techniques such as mindfulness, meditation, or gentle yoga may help reduce overall tension, which could alleviate some of your symptoms.

6. Hydration and Humidity: Ensure you are staying well-hydrated and consider using a humidifier in your living space, especially if you are in a dry environment. This can help keep your mucous membranes moist and may alleviate some discomfort.

In summary, persistent ear swelling and discomfort can stem from various causes, and it is crucial to continue seeking medical advice until you find a resolution. Keep a detailed record of your symptoms, including any changes or patterns, and communicate this information to your healthcare providers. This will help them make a more informed diagnosis and tailor a treatment plan that addresses your specific needs.
