No abdominal pain, but persistent diarrhea?

Hello Doctor, my father is nearly 50 years old and has had ongoing gastrointestinal issues.
Recently, he has been experiencing excessive stomach acid and vomiting a little after eating.
He underwent an endoscopy, and the doctor mentioned that his stomach is slightly inflamed but there are no major concerns.
Since July 17, he has been experiencing diarrhea for five days.
He has seen a doctor and taken anti-diarrheal medication, but there has been no improvement.
He reports that his stomach does not hurt, just feels a bit uncomfortable, but he continues to have diarrhea about 2-3 times a day.
He has lost around 2-3 kilograms due to the diarrhea and feels weak, unable to work.
His stool has been watery and yellow without any blood.
Could you please advise what might be causing this condition?

Kelly, 20~29 year old female. Ask Date: 2017/07/21

Dr. Chen Qifeng reply Gastroenterology and Hepatology

It is likely gastroenteritis; a low-fat, low-dairy, bland diet is recommended, and a visit to a gastroenterology outpatient clinic is advised.

Persistent diarrhea without abdominal pain can be a perplexing symptom, especially when accompanied by weight loss and fatigue. Given your father's history of gastrointestinal issues, including gastritis, it is essential to consider various potential causes for his current condition.

1. Infectious Causes: One of the most common reasons for diarrhea is an infection, which can be viral, bacterial, or parasitic. Even if your father does not experience abdominal pain, infections can still lead to diarrhea. Common viral infections include norovirus and rotavirus, while bacterial infections may arise from contaminated food or water, such as Salmonella or E. coli. Parasitic infections, such as Giardia, can also cause prolonged diarrhea. A stool test can help identify these pathogens.

2. Antibiotic-Associated Diarrhea: If your father has recently taken antibiotics for any reason, he may be experiencing antibiotic-associated diarrhea. This condition occurs when antibiotics disrupt the normal gut flora, leading to an overgrowth of harmful bacteria like Clostridium difficile (C. diff), which can cause severe diarrhea.

3. Inflammatory Bowel Disease (IBD): Conditions such as Crohn's disease or ulcerative colitis can present with diarrhea without significant abdominal pain, especially in the early stages. These conditions are characterized by inflammation of the gastrointestinal tract and can lead to symptoms like diarrhea, weight loss, and fatigue. A gastroenterologist can perform tests, including colonoscopy, to evaluate for IBD.

4. Irritable Bowel Syndrome (IBS): IBS is a functional gastrointestinal disorder that can cause diarrhea (IBS-D) without significant abdominal pain. Stress and dietary factors often exacerbate symptoms. While IBS is a diagnosis of exclusion, it is worth considering if other causes are ruled out.

5. Malabsorption Syndromes: Conditions that impair nutrient absorption, such as celiac disease or lactose intolerance, can lead to diarrhea. In celiac disease, ingestion of gluten triggers an immune response that damages the intestinal lining, resulting in diarrhea and weight loss. Lactose intolerance can cause diarrhea when dairy products are consumed. Blood tests and dietary trials can help diagnose these conditions.

6. Endocrine Disorders: Conditions such as hyperthyroidism can lead to increased bowel movements and diarrhea. If your father has symptoms like weight loss, increased appetite, or palpitations, thyroid function tests may be warranted.

7. Medication Side Effects: Certain medications can cause diarrhea as a side effect. If your father is taking any new medications, it is essential to review them with his healthcare provider.

Given the persistence of your father's diarrhea for over five days, along with weight loss and fatigue, it is crucial to seek further medical evaluation. A healthcare provider may recommend additional tests, including stool cultures, blood tests, and possibly imaging studies, to determine the underlying cause.
In the meantime, it is important for your father to stay hydrated, as diarrhea can lead to dehydration. Oral rehydration solutions can help replenish lost fluids and electrolytes. If his condition worsens or if he develops new symptoms, such as fever or severe weakness, immediate medical attention is necessary.

In summary, while there are many potential causes for persistent diarrhea without abdominal pain, a thorough evaluation by a healthcare professional is essential to identify the underlying issue and provide appropriate treatment.
