Just exerting force will result in urination?
Hello doctor, I have frequent urination, and the volume is quite large, always exceeding 300cc each time.
Sometimes, even when I feel like I have no urine left, I can still leak urine if I exert myself.
For example, after I finish urinating, if I contract my pelvic muscles and exert pressure, I can still release a little urine, and it takes at least five attempts to feel completely empty.
Additionally, I experience leakage when I sneeze.
Each time I urinate, I feel an urgent need and cannot urinate slowly; if I try to do so, it becomes uncomfortable.
I drink plenty of water, and there are bubbles in the toilet after I urinate.
What could be the issue?
A Gu, 20~29 year old female. Ask Date: 2017/07/25
Dr. Chen Jiansheng reply Urology
The symptoms include frequent urination, stress urinary incontinence, difficulty urinating, urgency, and suspected proteinuria.
Please visit the hospital for a urinalysis to rule out proteinuria, urinary tract infection, or other issues.

More Info
Frequent urination and involuntary leakage can be distressing symptoms that significantly impact one's quality of life. Based on the symptoms you've described, including the urgency to urinate, the sensation of incomplete emptying, and leakage upon exertion, several potential causes may be at play.
Possible Causes
1. Overactive Bladder (OAB): This condition is characterized by a sudden urge to urinate that may be difficult to control. It can lead to frequent urination, urgency, and sometimes leakage. OAB can occur without any underlying infection or other identifiable causes.
2. Urinary Tract Infection (UTI): Although you may not have mentioned any pain or burning during urination, a UTI can still cause increased frequency and urgency. It's essential to rule this out with a urine test.
3. Bladder Dysfunction: Conditions that affect the bladder's ability to store or empty urine properly can lead to symptoms like yours. This includes bladder inflammation, bladder stones, or even tumors.
4. Neurological Conditions: Issues with the nervous system, such as multiple sclerosis or spinal cord injuries, can affect bladder control and lead to symptoms of urgency and leakage.
5. Prostate Issues: In men, an enlarged prostate can cause urinary symptoms, including frequency, urgency, and incomplete emptying.
6. Diabetes: Uncontrolled diabetes can lead to increased urination due to high blood sugar levels. This condition can also cause nerve damage that affects bladder control.
7. Medications: Certain medications, especially diuretics, can increase urine production and lead to frequent urination.
Diagnostic Steps
To determine the underlying cause of your symptoms, a thorough evaluation is necessary. This may include:
- Urinalysis: To check for signs of infection, blood, or other abnormalities.
- Bladder Diary: Keeping track of your fluid intake and urination patterns can help identify triggers and patterns.
- Ultrasound: To assess bladder volume and check for any structural abnormalities.
- Urodynamic Testing: This can evaluate how well your bladder and urethra are functioning.
Treatment Options
Once a diagnosis is made, treatment can be tailored to your specific condition. Here are some common approaches:
1. Behavioral Therapies: Techniques such as bladder training and pelvic floor exercises can help improve bladder control and reduce urgency.
2. Medications: Anticholinergic medications can help relax the bladder and reduce urgency and frequency. Other medications may be prescribed based on the specific diagnosis.
3. Physical Therapy: Pelvic floor physical therapy can be beneficial, especially if leakage is related to pelvic floor dysfunction.
4. Lifestyle Modifications: Reducing caffeine and alcohol intake, managing fluid intake, and practicing timed voiding can help manage symptoms.
5. Surgery: In severe cases, surgical options may be considered, especially if there are anatomical issues contributing to the problem.
Conclusion
Your symptoms of frequent urination, urgency, and leakage are not uncommon, but they can be indicative of various underlying issues. It is crucial to consult a healthcare provider specializing in urology for a comprehensive evaluation and appropriate management. Early intervention can significantly improve your quality of life and help you regain control over your urinary function. Remember, you are not alone in this, and effective treatments are available to help manage your symptoms.
