Post-Circumcision Healing: Addressing Concerns and Recovery Tips - Urology

I would like to inquire about the healing process after circumcision?

Hello, doctor.
On June 9th, I underwent a laser circumcision due to recurrent inflammation of the foreskin for nearly six months, as assessed by my physician.
Today, seven weeks post-surgery, the wound has healed, and the swelling has mostly subsided.
Since I have a condition known as concealed penis due to obesity, the doctor mentioned that they couldn't remove too much foreskin during the procedure for fear that it would not be sufficient when fully erect.
Currently, when not erect, only half of the glans is exposed, but I can retract the foreskin completely to reveal the glans.
However, I have some concerns:
1.
The area around the wound is currently hard and lacks elasticity.
While it does not affect me when flaccid, during an erection, the lack of elasticity creates a tight sensation, as if being constricted by a rubber band, pushing the skin towards the glans.
Will this tightness gradually soften and regain elasticity over time?
2.
Since I still have some foreskin, should I regularly retract it to fully expose the glans?
3.
After the surgery, the doctor prescribed "New York Neomycin Ointment." Now that the wound has healed, should I continue to apply it?
I am currently working across the strait, so it will be a while before I can return to Taiwan to consult my attending physician.
Therefore, I am asking these questions here.
I am unsure if my description is complete or if I can attach images, so I am inquiring through text.
Thank you for your response, doctor.

Hello: 1.
All wounds will leave scars, but if there is inflammation, the scar tissue may become harder and tighter, requiring a longer time to soften.
Currently, keep the area clean, dry, and well-ventilated.
If there is moisture, make sure to dry it.
If the surrounding area is red, it indicates inflammation, so be cautious.
2.
It's sufficient to keep the glans partially covered.
If there is a slight erection, there is no need to worry, but avoid prolonged heat (wear looser underwear).
3.
If the skin is not red, there is no need for treatment, but if it is red, it may indicate inflammation, and applying an ointment would be advisable.

Post-circumcision healing can raise several concerns, especially regarding the appearance and functionality of the penis after surgery. It’s great to hear that your wound has healed and the swelling has subsided after your circumcision procedure. Here are some insights and recommendations based on your questions:
1. Tightness and Hardness of the Scar Tissue: It is common for the area around the circumcision site to feel tight or hard initially. This is due to scar tissue formation, which can be less elastic than the surrounding skin. Over time, scar tissue can soften and become more flexible, but this process can take several months. Gentle stretching exercises may help improve elasticity. However, it’s essential to be cautious and not cause any pain or discomfort while doing so. If the tightness persists or worsens, it would be advisable to consult a healthcare professional when you can.

2. Managing Remaining Foreskin: Since you still have some foreskin, it is generally recommended to retract it regularly to keep the glans (the head of the penis) exposed. This practice can help prevent any potential irritation or infection under the remaining foreskin. However, ensure that this is done gently and without causing discomfort. If you experience any pain or unusual symptoms while doing this, it would be best to seek medical advice.

3. Use of Neomycin Ointment: After the initial healing phase, the need for topical antibiotics like Neomycin may diminish. If your wound has healed well and there are no signs of infection (such as increased redness, swelling, or discharge), you may not need to continue using the ointment. However, if you notice any irritation or if the area becomes red or inflamed, it would be wise to apply the ointment as directed or consult a doctor for further advice.

In addition to addressing your specific questions, here are some general tips for post-circumcision care:
- Hygiene: Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid using harsh soaps or products that may irritate the skin.

- Moisturization: If the skin feels dry, consider using a gentle, fragrance-free moisturizer to keep the area hydrated. This can also help with the elasticity of the skin.

- Avoiding Irritation: Wear loose-fitting underwear and clothing to minimize friction and irritation in the area. This is especially important during the healing process.

- Monitoring for Complications: Keep an eye out for any signs of complications, such as increased pain, unusual discharge, or persistent swelling. If any of these occur, seek medical attention promptly.

- Follow-Up Care: Once you return to your home country, it’s essential to follow up with your surgeon or a urologist to discuss your healing process and any concerns you may have. They can provide tailored advice and reassurance based on your specific situation.

In conclusion, while it’s normal to have concerns post-surgery, many of the issues you’re experiencing can improve over time with proper care and attention. If you have any further questions or if your symptoms change, don’t hesitate to reach out to a healthcare professional when you have the opportunity.
