Issues with the surface of the glans?
Hello doctor, I have read a lot and asked questions, but it seems not many people have this issue.
I had phimosis for a long time, and recently I have been cleaning for almost a year, but there seems to be a white layer on the surface of the glans.
No matter how hard I scrub, and even when I use body wash, it doesn't seem to help.
Similar to this user, I have white patches on the glans, but mine covers almost the entire glans.
What should I do to resolve this situation? I have tried soaking in warm water and scrubbing hard, but it hasn't decreased.
Should I see a doctor? Would the doctor just think it's smegma and not examine it?

Dr. Du Shixi reply Urology
Hello: If it cannot be washed away, it is likely the natural color of the foreskin and glans.
It is not advisable to scrub it vigorously.
You may want to visit a urology department at a hospital, where the doctors will certainly assist you and provide recommendations.
Wishing you good health.

The presence of white bumps or a white coating on the glans (the head of the penis) can be concerning, and it's important to understand the potential causes and appropriate responses. Based on your description, it sounds like you may be experiencing a condition related to smegma, which is a combination of dead skin cells, oils, and moisture that can accumulate under the foreskin in uncircumcised males. This buildup can sometimes appear as a white, thick substance on the glans.
However, there are other possible causes for the white bumps or coating you are experiencing:
1. Smegma Accumulation: As mentioned, if you have a history of phimosis (tight foreskin), it can lead to the accumulation of smegma, which may appear as a white substance. Regular cleaning is essential, but if the foreskin is too tight, it may be difficult to clean effectively.
2. Balanitis: This is an inflammation of the glans, often caused by poor hygiene, infections (bacterial or fungal), or irritants. Symptoms may include redness, swelling, and discharge. If you notice any discomfort, itching, or unusual discharge, this could be a sign of balanitis.
3. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans. They are a normal anatomical variation and do not require treatment.
4. Lichen Sclerosus: This is a chronic skin condition that can cause white patches on the skin, including the genital area. It may require medical treatment if it causes discomfort or other symptoms.
5. Sexually Transmitted Infections (STIs): Some STIs can cause changes in the appearance of the genital area, including bumps or lesions. If you have had unprotected sexual contact, it is important to consider this possibility.
Given your situation, it is advisable to consult a healthcare professional, preferably a urologist or dermatologist, who can provide a thorough examination and accurate diagnosis. They may perform a physical examination and, if necessary, recommend tests to rule out infections or other conditions.
In the meantime, here are some general recommendations:
- Maintain Hygiene: Ensure that you are cleaning the area gently but thoroughly. If you are uncircumcised, gently retract the foreskin and wash with mild soap and water.
- Avoid Irritants: Be cautious with the use of soaps or lotions that may irritate the sensitive skin of the genital area.
- Monitor Symptoms: Keep track of any additional symptoms such as pain, itching, or changes in urination, as these can provide important information to your healthcare provider.
- Do Not Self-Diagnose: While it can be tempting to search for answers online, self-diagnosis can lead to unnecessary anxiety or inappropriate treatment. A professional evaluation is the best course of action.
In conclusion, while the appearance of white bumps or a coating on the glans can be related to various benign conditions, it is essential to seek medical advice to rule out any serious issues and receive appropriate treatment. Your health and comfort are paramount, and a healthcare provider can guide you toward the best solution for your specific situation.
