Dull pain below the navel?

Hello, I would like to ask about a discomfort I am experiencing.
Sometimes the dull pain is located directly below the navel, and other times it is above the navel, within the range of my palm.
The sensation feels somewhat like contractions, making me want to take deep breaths to relieve the pressure in my abdomen.
The duration of this discomfort varies, but it is consistently a dull pain, especially when I lie down in bed at night to prepare for sleep.
The pain can occur below the navel, sometimes on the right side, sometimes on the left side, or directly below.
This dull pain prevents me from sleeping.
At one point, I thought it might be a bladder issue, so I tried using the restroom, but there was no improvement after that.
This dull pain has persisted for three months, and recently it seems to be occurring more frequently.
I would like to know what might be causing this issue.
Also, how do you describe this dull pain in medical terms in English? Since I am currently abroad and unable to return to Taiwan for a consultation, I am unsure how to explain this dull, bloated sensation to a doctor in English.
Thank you! PS.
My bowel movements are normal without any pain or bleeding, and I do not have any symptoms like painful urination or abnormal vaginal discharge.
My menstrual cycle is also regular.

Dr. Yang Zhichao reply Gastroenterology and Hepatology

Dear Ms.
F, you don't need to use medical terminology; just describe the pain, including its duration and frequency.
How long does the pain last? Does it occur alongside pain in other areas? A dull pain is referred to as "dullness," while a sharp pain is called "sharp pain." You can see a doctor first; don't worry.

Persistent lower abdominal discomfort can be a challenging symptom to navigate, especially when it has been ongoing for several months. The sensation you describe, characterized as a "dull pain" or "pressure" in the lower abdomen, particularly around the area just below the belly button, can be attributed to a variety of causes.
First, it is essential to understand that abdominal pain can arise from various organs and systems, including the gastrointestinal tract, urinary system, reproductive organs, and even musculoskeletal structures. Given that your discomfort is localized and has a pattern of worsening at night, it may suggest a few potential underlying issues.

1. Gastrointestinal Causes: Conditions such as irritable bowel syndrome (IBS), constipation, or even gastritis can lead to persistent abdominal discomfort. IBS, in particular, is known for causing cramping and a sensation of bloating, which may feel like a tightening or pressure in the abdomen. If you have experienced changes in your bowel habits, such as increased frequency or consistency changes, this could be a contributing factor.

2. Urinary Tract Issues: Although you mentioned that you initially suspected a bladder issue, it is still worth considering. Conditions like interstitial cystitis or bladder inflammation can cause discomfort that may mimic abdominal pain. However, since you have not experienced any urinary symptoms such as pain during urination or blood in the urine, this may be less likely.

3. Gynecological Factors: In women, reproductive health issues such as ovarian cysts, endometriosis, or fibroids can lead to persistent lower abdominal pain. These conditions can cause cyclical pain or discomfort that may worsen at certain times of the month. Since you mentioned that your menstrual cycle is regular, this may reduce the likelihood of a gynecological cause, but it is still worth discussing with a healthcare provider.

4. Musculoskeletal Pain: Sometimes, abdominal pain can be referred from musculoskeletal issues, such as muscle strain or tension in the abdominal wall. This type of pain may feel like a tightening or pressure and can be exacerbated by certain positions, such as lying down.

5. Psychological Factors: Chronic pain can also have a psychological component. Stress and anxiety can manifest physically, leading to muscle tension and discomfort in the abdomen. If you have been experiencing increased stress or anxiety, this could be a contributing factor.

In terms of describing your symptoms to a healthcare provider in English, you might use terms like "dull pain," "pressure," "tightness," or "cramping" in the lower abdomen. You can say, "I have been experiencing persistent dull pain in my lower abdomen, particularly below my belly button. It feels like a pressure or tightness, especially when I lie down at night. It has been ongoing for three months, and it sometimes shifts to the left or right side."
Given the duration and persistence of your symptoms, it is advisable to seek medical evaluation. A healthcare provider may recommend imaging studies, such as an ultrasound or CT scan, to assess for any underlying conditions. Blood tests may also be useful to rule out infections or other systemic issues.
In conclusion, while there are several potential causes for your persistent lower abdominal discomfort, a thorough evaluation by a healthcare professional is essential to determine the exact cause and appropriate treatment. Keeping a symptom diary, noting when the pain occurs, its intensity, and any associated symptoms, can also be helpful for your healthcare provider in making a diagnosis.
