Breast issues

Hello doctor, when I press on the inner side of my right breast near the nipple, I experience pain that lasts about 3 to 4 seconds.
Sometimes it hurts when I press, and sometimes it doesn't.
Additionally, my right nipple is itchy, and when I scratch it, there are some flakes that come off.
What could be the issue? Thank you.

Bai, 20~29 year old female. Ask Date: 2017/08/11

Dr. Li Zhonghuai reply Surgery

Breast pain and itching can be concerning symptoms for many individuals, and understanding the potential causes is essential for addressing these issues effectively. Based on your description, there are several factors to consider regarding the pain and itching you are experiencing in your right breast, particularly near the nipple.

Possible Causes of Breast Pain
1. Hormonal Fluctuations: Breast pain, also known as mastalgia, is often linked to hormonal changes in the body, particularly those associated with the menstrual cycle. Many women experience breast tenderness or pain in the days leading up to their period due to increased levels of estrogen and progesterone. This type of pain is usually cyclical and may vary in intensity.

2. Fibrocystic Changes: This condition is characterized by lumpy, painful breasts due to the presence of cysts or fibrous tissue. It is common among women of reproductive age and can cause discomfort that may be exacerbated by hormonal changes.

3. Infections or Inflammation: Conditions such as mastitis, which is an infection of the breast tissue, can lead to localized pain and tenderness. While mastitis is more common in breastfeeding women, it can occur in non-lactating women as well. Symptoms may include redness, swelling, and warmth in the affected area.

4. Nipple Issues: The itching you describe, along with the presence of flakes or "dandruff," may indicate a condition affecting the nipple, such as eczema or dermatitis. These conditions can cause localized itching and discomfort and may require topical treatments.

5. Benign Breast Conditions: Other benign conditions, such as intraductal papillomas or duct ectasia, can also cause localized pain and discomfort. These conditions may require further evaluation through imaging or biopsy if a lump or persistent pain is noted.

Recommendations for Management
1. Self-Examination: Regular breast self-exams can help you become familiar with the normal texture and appearance of your breasts. If you notice any new lumps, changes in size, or persistent pain, it is essential to consult a healthcare provider.

2. Consult a Healthcare Provider: Given your symptoms, it would be prudent to schedule an appointment with a healthcare professional, such as a general surgeon or a breast specialist. They can perform a clinical examination and may recommend imaging studies, such as a mammogram or ultrasound, to rule out any serious conditions.

3. Topical Treatments: If your itching is due to a dermatological condition, over-the-counter hydrocortisone cream or moisturizers may help alleviate symptoms. However, it is best to consult a healthcare provider before starting any treatment.

4. Lifestyle Modifications: Wearing a supportive bra, avoiding caffeine, and managing stress can help reduce breast pain associated with hormonal fluctuations. Additionally, maintaining a healthy diet and staying hydrated can support overall breast health.

5. Monitoring Symptoms: Keep track of your symptoms, including when they occur, their intensity, and any associated changes in your menstrual cycle. This information can be valuable for your healthcare provider in determining the underlying cause.

Conclusion
Breast pain and itching can arise from various causes, ranging from hormonal changes to benign breast conditions. While many cases are benign and self-limiting, it is essential to seek medical advice for persistent or concerning symptoms. Early evaluation and appropriate management can help alleviate discomfort and address any underlying issues effectively. Remember, your health and peace of mind are paramount, so do not hesitate to reach out to a healthcare professional for guidance.
