Frequent urination without pain and without discharge?

Hello doctor, two weeks ago I started feeling the urge to urinate frequently, but there is no pain or burning sensation during urination, and I haven't had any discharge (the only time I noticed was on the fourth or fifth day, when I saw a very small amount of green discharge, about the size of a mung bean, and nothing since then).
As soon as I felt the frequent urination, I went to the Kunming Prevention and Treatment Center, where the doctor prescribed antibiotics, and I have been taking them for two weeks now.
I am about to enter the third week of antibiotics, and I have not had any sexual activity during this time.
After considering whether stress might be causing autonomic nervous system dysregulation, I consulted a psychiatrist and took medication for a week, but there has been no significant improvement.
Currently, I feel a constant sensation of fullness and itching in the genital area, with the urge to urinate, but pressing on the bladder does not elicit any response.
I do not go to the bathroom very often, but when I do, the volume is low, probably less than 100cc.
My primary physician has also noted that I do not have pain or discharge, and after two weeks of antibiotics, there has been no noticeable effect.
I would like to ask if it is possible that I have some kind of disease, and how should I go about getting tested? Thank you.

Hello: The main thing is to assess your test results.
If your urine and blood tests are normal, you should pay attention to your lifestyle, hydration, exercise, and avoid certain beverages.
Improvement will take some time.

More Info

Frequent urination without pain or discharge can be a perplexing symptom, and it’s understandable that you are seeking clarity on this issue. Based on your description, there are several potential causes and avenues for further investigation.

Possible Causes
1. Overactive Bladder (OAB): This condition is characterized by a sudden urge to urinate frequently, often without the presence of pain or discomfort. It can occur due to involuntary contractions of the bladder muscle.

2. Interstitial Cystitis (IC): Also known as painful bladder syndrome, IC can cause frequent urination and a feeling of pressure or discomfort in the bladder area, but it may not always present with pain during urination.

3. Urinary Tract Infection (UTI): While you mentioned that you have been on antibiotics for two weeks without improvement, it’s still worth considering that a UTI could be present but not detected in standard urine tests. Some infections can be resistant to common antibiotics.

4. Prostatitis: In males, inflammation of the prostate can lead to urinary symptoms, including frequent urination without pain. This condition can sometimes present with minimal or no pain.

5. Psychogenic Factors: Stress and anxiety can significantly impact urinary habits. The sensation of needing to urinate frequently can be exacerbated by psychological factors, leading to a condition known as psychogenic polydipsia or anxiety-related urinary frequency.

6. Bladder or Kidney Stones: Though less likely without pain, small stones can sometimes cause irritation to the bladder, leading to frequent urination.

Recommended Tests
Given your symptoms and the lack of improvement with antibiotics, it would be prudent to consider the following tests:
1. Urinalysis and Urine Culture: A thorough urinalysis can help rule out infections, while a culture can identify specific bacteria if an infection is present.

2. Ultrasound of the Kidneys and Bladder: This imaging test can help visualize any abnormalities, such as stones or structural issues in the urinary tract.

3. Cystoscopy: This procedure involves inserting a thin tube with a camera into the bladder to directly visualize the bladder lining and check for signs of inflammation or other abnormalities.

4. Urodynamic Testing: This series of tests assesses how well the bladder and urethra are storing and releasing urine, which can help diagnose conditions like OAB or IC.

5. Blood Tests: Checking for kidney function and other systemic issues can provide additional insights into your urinary symptoms.

Management Strategies
While awaiting further tests, consider the following management strategies:
- Hydration: Ensure you are adequately hydrated, as concentrated urine can irritate the bladder.

- Dietary Modifications: Avoid irritants such as caffeine, alcohol, and spicy foods, which can exacerbate bladder symptoms.

- Pelvic Floor Exercises: Strengthening the pelvic floor muscles can sometimes help with urinary frequency.

- Stress Management: Techniques such as mindfulness, yoga, or cognitive behavioral therapy can help if anxiety is contributing to your symptoms.

Conclusion
In summary, frequent urination without pain or discharge can stem from various causes, including overactive bladder, interstitial cystitis, or psychological factors. Given your ongoing symptoms despite antibiotic treatment, it is essential to pursue further diagnostic testing to identify the underlying issue. Collaborate closely with your healthcare provider to explore these options and develop a tailored management plan. Your health and comfort are paramount, and addressing these symptoms is crucial for your overall well-being.
