The surface of the glans is white instead of the normal pink color?
Hello doctor, I have phimosis but I can still retract it fully.
However, the glans is white, and washing it hasn’t improved the situation (I only use my hands to clean, without any body wash).
After washing, I dry it with a towel before covering it back.
I consulted a doctor who said it’s normal and that everyone can clean it, but I just can’t remove the white part! He mentioned it might be a fungal infection and prescribed me Biquan ointment from the Kwang Hwei brand, but there has been no improvement after using it.
I found online that it might be "leukoplakia"? However, there is no pain, itching, or odor.
If it really is leukoplakia, will it not improve? How can I restore the normal color of the glans?

Hello: Vitiligo is unlikely to improve.
The color of the glans varies from person to person; some may have a reddish tint, others a whitish hue, some may appear darker, and others may have a mottled appearance.
Based on personal experience, the chances of returning to a normal color are very slim.
Wishing you good health.

More Info
Understanding white discoloration on the glans, particularly in the context of phimosis, can be a source of concern for many individuals. The condition you describe, where the glans appears white and does not improve with washing, can be attributed to several potential causes, including but not limited to, physiological changes, infections, or dermatological conditions.
Causes of White Discoloration on the Glans
1. Physiological Changes: In some cases, the glans may appear white due to a lack of exposure to air and moisture, especially in individuals with phimosis (a condition where the foreskin cannot be fully retracted). This can lead to a buildup of keratin, which may give the glans a whitish appearance.
2. Fungal Infections: Your doctor mentioned a possible fungal infection. Conditions like candidiasis (a yeast infection) can cause white patches on the glans. This is often accompanied by symptoms such as itching or discomfort, but in some cases, it may be asymptomatic.
3. Balanitis: This is an inflammation of the glans, which can be caused by irritants, infections, or poor hygiene. It can lead to discoloration and may require specific treatments depending on the underlying cause.
4. Lichen Sclerosus: This is a chronic skin condition that can affect the genital area, leading to white patches and changes in skin texture. It is often painless but can lead to complications if left untreated.
5. Leukoplakia: This condition is characterized by white patches in the mucous membranes and can be a sign of irritation or a precancerous condition. It is less common but should be evaluated by a healthcare professional.
Solutions and Treatment Options
1. Proper Hygiene: Continue to maintain good hygiene by gently cleaning the area with water. Avoid using harsh soaps or irritants that could exacerbate the condition.
2. Topical Treatments: If a fungal infection is suspected, antifungal creams (like the one prescribed) should be used as directed. If there is no improvement, it may be necessary to revisit your healthcare provider for further evaluation.
3. Dermatological Evaluation: If the discoloration persists despite treatment, consider seeing a dermatologist who specializes in skin conditions. They may perform a biopsy or other diagnostic tests to determine the exact cause.
4. Avoid Irritants: Steer clear of potential irritants such as scented soaps, lotions, or other products that may cause inflammation or allergic reactions.
5. Follow-Up: Regular follow-ups with your healthcare provider are essential, especially if symptoms change or worsen. Early intervention can prevent complications.
Prognosis
If the condition is due to a benign cause, such as physiological changes or a mild fungal infection, it may resolve with appropriate treatment. However, if it is due to a more chronic condition like lichen sclerosus or leukoplakia, ongoing management may be necessary.
Conclusion
While the white discoloration on the glans can be concerning, many cases are manageable with proper care and treatment. It is crucial to follow your healthcare provider's recommendations and seek further evaluation if symptoms persist or worsen. Remember, open communication with your healthcare provider about your concerns is vital for effective management and peace of mind.
