Erectile dysfunction concerns?

Hello doctor, I am 24 years old.
Before I started dating my current girlfriend, I had issues with insufficient erection hardness during masturbation, which led to quick loss of erection after ejaculation or becoming soft with just a slight distraction.
This situation has persisted for a long time.
Later, I visited a urologist who conducted blood tests, and my testosterone levels were within the normal range, with no diabetes diagnosed.
The doctor advised me to exercise more and observe the situation.
After some time, I started dating my girlfriend, but due to ongoing erectile dysfunction, I purchased erectile enhancement medication from a pharmacy.
The first and second times were normal during intercourse, but on the third occasion, when I tried to use a condom, I lost my erection and couldn’t maintain it.
In subsequent attempts, I also struggled to achieve an erection during foreplay, despite taking the enhancement medication.
I then visited another clinic, where I had blood tests again, and my testosterone levels were still normal.
The doctor prescribed me with Cialis, and after a week, I returned for a follow-up, but the results were unsatisfactory.
The doctor suggested I try a lower dose of Viagra.
However, due to certain circumstances, I had to separate from my girlfriend.
I continued taking the medication, and while I noticed some improvement in maintaining an erection during masturbation, I am unsure how it would perform during actual intercourse.
After stopping the medication, I feel like my condition has not improved.
Recently, I have been experiencing lower back pain, soreness, and numbness in my legs.
A rehabilitation doctor diagnosed me with an L5/S1 herniated disc.
I would like to ask if my persistent erectile dysfunction, which I have been unable to resolve, could be related to my back pain.
I have felt inadequate hardness and quick loss of erection for a long time during masturbation.
Occasionally, I do experience nocturnal erections when I touch myself while half-asleep, but they are not very firm, and usually fade after waking up.
There have been a few instances where I had a firm erection, but it quickly went soft as soon as I got up.
I would appreciate any insights you may have, as I am eager to resolve this issue at just 24 years old.
Thank you.

Hello: Your condition requires an examination by a specialized urologist.
There are many causes of erectile dysfunction, and since general tests have been conducted and returned normal, it is advisable to consult a urologist at a specialized hospital (some have expertise in sexual function).
If there are no issues with masturbation, it is likely to be psychological, but confirmation from a specialist is necessary.
Wishing you good health.

More Info

Erectile dysfunction (ED) in young men, particularly those in their 20s, can be a distressing issue, and it's important to understand the potential causes and treatment options available. Based on the information provided, several factors could contribute to your situation.

Causes of Erectile Dysfunction in Young Men
1. Psychological Factors: One of the most common causes of ED in younger men is psychological. Anxiety, stress, and depression can significantly impact sexual performance. The pressure to perform, especially in new relationships, can lead to performance anxiety, which may exacerbate the problem.
2. Lifestyle Choices: Irregular sleep patterns, poor diet, lack of exercise, and substance use (like smoking or excessive alcohol consumption) can all contribute to ED. In your case, you mentioned irregular sleep and possibly inadequate nutrition, which can affect overall health and sexual function.

3. Physical Health Issues: Although you have reported normal testosterone levels and no diabetes, other physical health issues could still play a role. Conditions such as obesity, cardiovascular problems, or hormonal imbalances can affect blood flow and nerve function, leading to ED. Your diagnosis of a herniated disc (L5/S1) could also be a contributing factor, as back pain can affect nerve signals and blood flow to the pelvic region.

4. Medication Side Effects: If you are taking medications for any health issues, including pain management for your back, these could potentially have side effects that impact erectile function.

Treatment Options
1. Lifestyle Modifications:
- Exercise: Regular physical activity can improve blood flow, reduce stress, and enhance overall health. Aim for at least 150 minutes of moderate aerobic activity each week.

- Diet: A balanced diet rich in fruits, vegetables, whole grains, and lean proteins can improve overall health and potentially enhance erectile function.

- Sleep: Prioritize getting adequate sleep each night. Aim for 7-9 hours of quality sleep to help regulate hormones and reduce stress.

2. Psychological Support: If anxiety or stress is a significant factor, consider speaking with a mental health professional. Cognitive-behavioral therapy (CBT) can be particularly effective in addressing performance anxiety and other psychological barriers to sexual function.

3. Medical Treatment: Since you have already tried medications like sildenafil (Viagra) and found some temporary relief, it might be worth discussing with your healthcare provider about adjusting the dosage or trying different medications. However, it’s essential to use these medications under medical supervision, especially considering your back pain and any other health issues.

4. Addressing Physical Health: Continue to monitor your back pain and work with your healthcare provider on a rehabilitation plan. Managing your back pain effectively may help improve your overall physical condition and sexual function.

5. Regular Check-ups: Since you have undergone blood tests and other evaluations, keep in touch with your healthcare provider to monitor any changes in your condition. If symptoms persist, further evaluations, such as a penile Doppler ultrasound, may be warranted to assess blood flow and vascular health.

Conclusion
Erectile dysfunction at a young age can be multifactorial, involving psychological, lifestyle, and physical health components. It’s crucial to approach this issue holistically, addressing both the mental and physical aspects. By making lifestyle changes, seeking psychological support, and working closely with your healthcare provider, you can improve your situation. Remember, you are not alone in this, and many young men experience similar challenges. Open communication with your partner and healthcare providers can also alleviate some of the stress associated with this condition.
