Heavy menstrual bleeding?
I am not sure if the heavy menstrual flow is related to the insertion of the intrauterine device (IUD).
Should I consider having it removed? It has been over ten months since I had the IUD placed after giving birth, and my menstrual flow has been very heavy, requiring me to change sanitary pads approximately every half hour.
Additionally, I am experiencing lower back pain and anemia.
I am unsure of what to do and would appreciate your advice, Doctor.
I have three children already; is a vasectomy a viable option? Would it require general anesthesia or local anesthesia? Thank you, Dr.

Dr. Zhan Deqin reply Obstetrics and Gynecology
Yes, contraceptive devices can increase menstrual flow.
Sterilization is a good option, but it typically requires general anesthesia and is an out-of-pocket expense.
If you have further questions, it is recommended to visit an obstetrics and gynecology clinic.

Heavy menstrual bleeding after IUD insertion can be a concerning issue for many women. It is not uncommon for some women to experience heavier periods after the insertion of an intrauterine device (IUD), particularly in the first few months following the procedure. However, if the bleeding is excessive, as you describe—requiring a change of sanitary products every half hour and accompanied by symptoms such as lower back pain and anemia—this situation warrants further evaluation.
Firstly, it's important to understand that IUDs can affect menstrual bleeding in different ways depending on the type of IUD used. Hormonal IUDs, such as Mirena, often lead to lighter periods over time, while copper IUDs can sometimes cause heavier menstrual bleeding and increased cramping. If you have been experiencing heavy bleeding consistently for ten months, it may be worth discussing with your healthcare provider whether the type of IUD you have is contributing to your symptoms.
Given your symptoms of heavy bleeding and anemia, it is advisable to schedule an appointment with your healthcare provider. They may perform a physical examination, possibly including an ultrasound, to ensure that the IUD is properly positioned and to rule out any other underlying conditions such as fibroids or polyps that could be contributing to your heavy bleeding. Additionally, they can assess your hemoglobin levels to evaluate the extent of your anemia and recommend appropriate treatment, which may include iron supplements or dietary changes.
Regarding your consideration of having the IUD removed, this is certainly an option if the bleeding is impacting your quality of life. Your healthcare provider can discuss the potential benefits and drawbacks of removal, as well as alternative contraceptive methods that may be more suitable for you.
As for your question about sterilization (tubal ligation), this is a permanent form of contraception that can be performed postpartum or at a later date. The procedure can be done under general anesthesia or local anesthesia, depending on the method used and your healthcare provider's recommendations. It's essential to have a thorough discussion with your provider about your family planning goals and any concerns you may have regarding the procedure.
In summary, heavy menstrual bleeding after IUD insertion is a valid concern that should be addressed with your healthcare provider. They can help determine the cause of your symptoms and discuss potential solutions, including the possibility of IUD removal or alternative contraceptive options. Additionally, addressing your anemia is crucial for your overall health and well-being. Don't hesitate to seek medical advice to ensure that you receive the appropriate care and support.
