Penile white membrane stretching
Hello doctor, I have recently been experiencing a curvature of the penis that is not congenital.
It started to bend around July, and it is quite noticeable when it is semi-erect, curving to the left (it did not do this before July).
However, when fully erect, it straightens out, and I do not feel any pain or have experienced any trauma.
I have consulted other doctors, but they did not perform an ultrasound; they only prescribed medication for peripheral blood circulation.
Could this condition be due to abnormal tunica albuginea stretching? Is surgery necessary, and what are the chances of spontaneous recovery?
An An, 20~29 year old female. Ask Date: 2017/09/26
Dr. Xu Wencang reply Urology
The assessment should be based on a full erection; if it is straight, further observation can be conducted.

More Info
Understanding penile curvature, particularly when it develops later in life rather than being congenital, can be concerning for many individuals. The condition you are describing, where the penis curves to one side during a semi-erect state but straightens when fully erect, could be indicative of a few potential underlying issues, including Peyronie's disease, which is characterized by the formation of fibrous scar tissue inside the penis that can cause curved, painful erections.
Causes of Penile Curvature
1. Peyronie's Disease: This is the most common cause of acquired penile curvature. It typically occurs due to trauma or injury to the penis, which may not always be remembered or recognized by the individual. The injury leads to scar tissue formation, which can cause the penis to bend during erections.
2. Fibrosis: Conditions that lead to fibrosis or scarring of the penile tissue can also result in curvature. This can be due to various factors, including inflammation or other medical conditions.
3. Genetic Factors: While you mentioned that this curvature is not congenital, some individuals may have a genetic predisposition to develop conditions like Peyronie's disease.
4. Other Medical Conditions: Certain systemic diseases, such as diabetes or Dupuytren's contracture, can also be associated with penile curvature.
Diagnosis
Diagnosis typically involves a thorough medical history and physical examination. While some doctors may not perform an ultrasound, it can be a useful tool to assess the extent of any fibrous tissue or plaque formation. Other diagnostic methods may include:
- Penile Doppler Ultrasound: This test evaluates blood flow and can help identify any abnormalities in the penile tissue.
- X-rays or MRI: In some cases, imaging may be used to assess the structure of the penis and surrounding tissues.
Treatment Options
1. Observation: If the curvature is not causing pain or significant functional issues, some doctors may recommend a wait-and-see approach, as Peyronie's disease can sometimes stabilize or improve on its own.
2. Medications: There are medications available that may help reduce the curvature or improve symptoms. These can include oral medications or injections directly into the plaque.
3. Vacuum Devices: These devices can help improve blood flow and may assist in straightening the penis over time.
4. Surgery: Surgical options are typically considered when the curvature is severe, causing pain, or significantly affecting sexual function. Surgical procedures may involve removing the plaque or shortening the longer side of the penis to correct the curvature.
5. Penile Traction Therapy: This is a non-invasive method that involves stretching the penis to help reduce curvature over time.
Prognosis and Self-Healing
The chances of self-healing vary from person to person. Some individuals may experience spontaneous improvement, while others may require medical intervention. Early diagnosis and treatment can significantly affect the outcome, so it is essential to consult with a urologist who specializes in penile disorders for a comprehensive evaluation and tailored treatment plan.
Conclusion
In summary, penile curvature can arise from various causes, and while it may not always require surgical intervention, it is crucial to seek a thorough evaluation from a specialist. If you are experiencing significant changes or discomfort, do not hesitate to pursue further diagnostic testing and treatment options. Your sexual health and well-being are important, and there are effective strategies to address these concerns.
