Dull pain in the lower abdomen after ejaculation?

Hello Doctor: After sexual intercourse, within 5 minutes, I start to experience a "slow" onset of dull pain in my lower abdomen (not a sudden sharp pain).
I have a strong urge to urinate, but there is nothing to pass.
Both my urethra and lower abdomen feel very uncomfortable and congested.
Then, within 30 minutes, it gradually eases.
Could you please tell me what the issue might be? (I have experienced the same symptoms about three times, and it is not random.)

Niming, 30~39 year old female. Ask Date: 2017/10/19

Dr. Du Shixi reply Urology

Hello: Ejaculation is mostly related to the prostate.
It is recommended to walk for half an hour every day and to limit the intake of spicy and irritating foods.
If the issue persists, medical attention should be sought.
Wishing you good health.

Post-ejaculation lower abdominal discomfort can be a concerning symptom, and it is important to understand the potential causes and implications of this condition. The discomfort you are experiencing, which occurs shortly after ejaculation and is characterized by a dull, persistent pain in the lower abdomen, along with a sensation of urgency to urinate, can be attributed to several factors.

One possible cause is related to the physiological changes that occur during sexual arousal and ejaculation. During sexual activity, the pelvic muscles contract, and this can lead to temporary discomfort in the lower abdomen. After ejaculation, the body undergoes a series of changes, including the release of tension in the pelvic muscles, which may result in a sensation of discomfort or pressure in the area. This is often a normal response and may not indicate any underlying pathology.

Another potential cause could be related to the urinary system. The sensation of needing to urinate without being able to do so could suggest that there is some irritation or inflammation in the urinary tract. Conditions such as prostatitis (inflammation of the prostate gland) or urethritis (inflammation of the urethra) can lead to discomfort in the lower abdomen and urinary urgency. These conditions can be caused by infections, sexually transmitted infections, or other irritants.

Additionally, it is worth considering the possibility of pelvic floor dysfunction. The pelvic floor muscles support various organs, including the bladder and prostate. If these muscles are overly tense or not functioning properly, they can contribute to discomfort during and after sexual activity. This dysfunction can sometimes be associated with anxiety or stress related to sexual performance or other factors.

If this discomfort persists or worsens, it is essential to seek medical evaluation. A healthcare provider may perform a thorough examination, including a review of your medical history, a physical examination, and possibly laboratory tests or imaging studies to rule out any underlying conditions. It is particularly important to rule out infections or other issues that may require treatment.

In terms of management, if the discomfort is determined to be related to muscle tension or pelvic floor dysfunction, pelvic floor physical therapy may be beneficial. This type of therapy focuses on strengthening and relaxing the pelvic muscles, which can alleviate discomfort and improve overall function. Additionally, practicing relaxation techniques and stress management may help reduce any anxiety that could be contributing to the symptoms.

In conclusion, while post-ejaculation lower abdominal discomfort can be a common experience, it is important to monitor the symptoms and seek medical advice if they persist or are accompanied by other concerning signs, such as fever, changes in urinary habits, or significant pain. Early intervention can help identify any underlying issues and ensure appropriate management.
