Postmenopausal Vaginal Dryness Care

Hello, Director Cao.
I am a breast cancer patient who has been taking Replens for four years.
After my breast cancer treatment, I have been completely menopausal, and I underwent an oophorectomy three years ago.
Recently, I have been experiencing significant vaginal pain after showering.
I recalled that my surgeon had recommended a French ointment, Evo Comfort Gel, Tube, for me to use after my ovarian surgery.
At that time, I did not use it because my vaginal dryness was not severe.
However, due to the recent increase in vaginal pain, I decided to try it, and it has worked quite well.
When my doctor recommended the ointment, they specifically chose this one because it does not contain any hormones, knowing that I am a breast cancer patient.
Unfortunately, this ointment is no longer available for sale in Taiwan.
I would like to ask if you have any recommendations for hormone-free ointments that are available for purchase in Taiwan.
Thank you.

Ms. Hu, 40~49 year old female. Ask Date: 2017/10/19

Dr. Cao Guotao reply Obstetrics and Gynecology

Post-menopausal vaginal dryness is a common issue faced by many women, especially those who have undergone treatments for breast cancer or have had surgical procedures such as oophorectomy (ovary removal). The hormonal changes that occur after menopause can lead to a decrease in vaginal lubrication, resulting in discomfort, pain during intercourse, and other related symptoms. Given your history as a breast cancer patient and your current concerns, it is essential to find safe and effective solutions that do not involve hormone replacement therapy (HRT).

Since you mentioned that you have had success with the Evo Comfort Gel, which is hormone-free, it’s unfortunate that it is no longer available in Taiwan. However, there are several alternatives that you can consider. Here are some hormone-free vaginal moisturizers and lubricants that are generally well-tolerated and can be found in pharmacies or online:
1. Water-based lubricants: These are widely available and can provide immediate relief from dryness. Brands like Astroglide, KY Jelly, and Good Clean Love offer water-based options that are safe for vaginal use and do not contain hormones.

2. Silicone-based lubricants: These tend to last longer than water-based lubricants and can provide a smoother experience. Products like Pjur and Wet Platinum are popular choices. They are also safe for use with condoms.

3. Vaginal moisturizers: Products like Replens and Vagisil offer long-lasting moisture and can be used regularly to help alleviate dryness. They are designed to be used a few times a week and can help maintain vaginal moisture over time.

4. Natural oils: Some women find relief using natural oils such as coconut oil or olive oil. These can be applied externally or internally (if you are not sensitive to them) and can provide lubrication without the use of hormones.

5. Aloe Vera Gel: This natural product is known for its soothing properties and can be used as a vaginal moisturizer. Ensure that you choose a pure aloe vera gel without added fragrances or chemicals.

In addition to these products, there are some lifestyle changes and care tips that may help alleviate symptoms of vaginal dryness:
- Stay Hydrated: Drinking plenty of water can help maintain overall hydration, which may positively affect vaginal moisture levels.

- Avoid Irritants: Be cautious with soaps, douches, and other products that may irritate the vaginal area. Opt for gentle, unscented products.

- Regular Sexual Activity: Engaging in sexual activity can promote blood flow to the vaginal area and may help maintain elasticity and lubrication.

- Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can improve vaginal health and may help with dryness.

- Consult with a Healthcare Provider: Since you have a history of breast cancer, it’s crucial to discuss any new treatments or products with your healthcare provider to ensure they are safe for your specific situation.

In conclusion, while hormonal treatments are often effective for managing post-menopausal vaginal dryness, there are numerous hormone-free alternatives available. It’s essential to find a product that works for you and to maintain open communication with your healthcare provider regarding your symptoms and treatment options. Your comfort and health are paramount, and with the right approach, you can manage vaginal dryness effectively.
