Anal issues
Hello doctor, I had an anal abscess drainage surgery six months ago.
Due to my body constitution, a keloid formed after the wound healed.
Recently, the keloid area, which had been asymptomatic, suddenly protruded and is painful to the touch, although it feels soft and resembles a blister.
I am currently working abroad and am a bit worried that this might indicate the abscess has turned into a fistula.
Could you please advise on how to alleviate this condition?

Dr. Ke Fangxu reply Surgery
Your condition may be an anal fistula complicated by a bacterial infection.
If immediate surgical treatment is not possible, you should start oral antibiotics.
If you are unable to obtain antibiotics from a pharmacy, you should seek medical attention immediately to prevent the infection from worsening and leading to sepsis.

After undergoing surgery for an anal abscess, it is not uncommon to experience various skin changes and discomfort during the healing process. The development of a keloid, or crab foot scar, as you described, can occur due to your body's unique healing response. Keloids are raised scars that form when the body produces too much collagen during the healing process. They can be itchy, painful, and may change in size or shape over time.
In your case, the sudden change in the keloid, where it has become more prominent and painful to the touch, raises some concerns. While it is possible that this is simply a manifestation of your body's healing process, it is also important to rule out any complications such as the formation of a fistula, which can occur after an anal abscess. A fistula is an abnormal connection between the anal canal and the skin, which can lead to recurrent infections and discomfort.
To manage your symptoms and alleviate pain, consider the following strategies:
1. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help reduce pain and inflammation. Always follow the recommended dosages and consult with a healthcare professional if you have any concerns or pre-existing conditions.
2. Topical Treatments: Applying a topical corticosteroid cream may help reduce inflammation and itching associated with the keloid. Additionally, silicone gel sheets or silicone ointments can be effective in flattening keloids over time. These products can help hydrate the scar tissue and may reduce its appearance.
3. Avoiding Irritation: It is crucial to avoid any activities that may irritate the area, such as tight clothing or excessive friction. Keeping the area clean and dry can also help prevent further irritation.
4. Consulting a Specialist: Since you are currently abroad, it may be beneficial to seek out a local healthcare provider who specializes in dermatology or wound care. They can assess the keloid and determine if any further treatment is necessary, such as corticosteroid injections, laser therapy, or surgical options to remove the keloid if it becomes problematic.
5. Monitoring for Complications: Keep an eye on the area for any signs of infection, such as increased redness, swelling, warmth, or discharge. If you notice any of these symptoms, it is essential to seek medical attention promptly.
6. Follow-Up Care: Once you return home, ensure you have a follow-up appointment with your surgeon or a specialist to discuss your concerns and the best course of action for your healing process.
In summary, while some discomfort and changes in the skin after anal abscess surgery can be normal, it is essential to monitor your symptoms closely. If the pain persists or worsens, or if you have concerns about the possibility of a fistula, seeking medical advice is crucial. Proper management and care can help alleviate your symptoms and promote better healing.
