Do antibiotics affect the menstrual cycle?

Hello Doctor, I have been taking antibiotics (cephalexin monohydrate 500mg) for about two weeks due to a urinary tract infection since October 6th.
My period was expected around October 23rd, but it still hasn't arrived as of today, October 30th.
It has usually been late by only four to five days, so I am wondering if it could be related to the medication.

JAky, 10~19 year old female. Ask Date: 2017/10/30

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello Miss Jaky: The menstrual cycle in women can sometimes be affected by emotions, stress, illness, or medication.
The delay in your menstrual period could be due to the illness itself or the medication.
However, we should not overlook another possibility, which is pregnancy.
If this possibility cannot be ruled out, I recommend that you take a urine test.

Antibiotics, including cephalexin, are commonly prescribed for various infections, including urinary tract infections like cystitis. While antibiotics are effective in treating bacterial infections, their impact on the menstrual cycle is a topic of interest for many patients.
Firstly, it is essential to understand that antibiotics do not directly affect the menstrual cycle in most cases. The menstrual cycle is primarily regulated by hormonal changes in the body, particularly involving the hypothalamus, pituitary gland, and ovaries. Antibiotics do not typically interfere with these hormonal pathways. However, there are a few indirect ways in which antibiotics might influence your menstrual cycle.

1. Gut Flora and Hormonal Balance: Antibiotics can disrupt the natural balance of gut bacteria. The gut microbiome plays a role in the metabolism of hormones, including estrogen. A significant alteration in gut flora could potentially lead to changes in hormone levels, which might affect the menstrual cycle. However, this is more theoretical and not commonly observed in clinical practice.

2. Stress and Illness: The stress of dealing with an infection, the physical symptoms of illness, and the psychological impact of being unwell can also affect your menstrual cycle. Stress can lead to hormonal imbalances that may delay menstruation. If you have been feeling unwell or stressed due to your bladder infection, this could be a contributing factor to your delayed period.

3. Other Medications: If you are taking other medications alongside antibiotics, these could also influence your menstrual cycle. For instance, some medications can affect hormonal balance or interact with hormonal contraceptives, leading to changes in your cycle.

4. Underlying Health Conditions: If you have any underlying health conditions that affect your menstrual cycle, such as polycystic ovary syndrome (PCOS) or thyroid disorders, these could also play a role in the timing of your period.
In your case, since you have been taking cephalexin for about two weeks and your period is delayed by a week, it is unlikely that the antibiotic itself is the direct cause of your missed period. It is more plausible that stress from the infection or other factors may be contributing to this delay.
If your period does not arrive soon, or if you experience other concerning symptoms such as severe pain, unusual discharge, or if you suspect you might be pregnant, it would be wise to consult your healthcare provider. They can perform a thorough evaluation, including a pregnancy test if necessary, and help determine if further investigation is needed.

In summary, while antibiotics like cephalexin are not typically known to affect menstrual cycles directly, various indirect factors related to health, stress, and hormonal balance can contribute to changes in your cycle. If you have concerns about your menstrual health or the effects of medications, discussing these with your doctor is always a good approach.
