Itchy sensation inside the nipple?
I underwent a total mastectomy for in situ carcinoma, and since around the time of my surgery in May, I have been experiencing an itchy sensation inside the nipple (not on the surface).
Sometimes the itching extends to surrounding areas, but an ultrasound at the Veterans General Hospital showed nothing abnormal.
What could this be? The itching in the nipple makes me feel like it could be a sign of cancer, doesn't it? Thank you!

Dr. Ke Fangxu reply Surgery
It is possible for cancer to cause nipple itching, but not all cases of nipple itching are due to breast cancer; most often, it is due to dermatitis of the nipple skin.
Over-the-counter anti-itch creams can be used for treatment.
However, if there is associated skin breakdown or abnormal discharge, it may indicate a more serious issue.
If you are regularly undergoing check-ups, there should be no major concerns.
You can start with topical ointments, but if the symptoms persist without improvement, it is advisable to consult a physician for further evaluation.

More Info
Itchy nipples, especially after surgery such as a mastectomy, can be concerning for many patients. However, it is essential to understand that itchiness in this area does not necessarily indicate cancer or a serious underlying condition. Let's explore the potential causes and concerns regarding itchy nipples post-surgery, as well as when to seek further medical advice.
Potential Causes of Itchy Nipples After Surgery
1. Healing Process: After any surgical procedure, the body goes through a healing phase. This can include nerve regeneration and changes in skin sensitivity. Itching can be a normal part of the healing process as the nerves and skin adjust post-surgery.
2. Dry Skin: Surgical procedures can lead to changes in skin hydration. If the skin around the nipple area becomes dry, it can cause itching. This can be exacerbated by factors such as medications, changes in body chemistry, or even environmental conditions.
3. Scar Tissue Formation: Following surgery, scar tissue may form, which can sometimes lead to sensations of itching or discomfort. This is particularly true if the surgery involved significant tissue removal or alteration.
4. Allergic Reactions: Sometimes, patients may develop sensitivities or allergic reactions to topical medications, dressings, or even laundry detergents used on clothing that comes into contact with the surgical area.
5. Infection: Although you mentioned that an ultrasound showed no issues, it is still important to be aware that infections can sometimes present with itching, redness, or swelling. If there are any signs of infection, such as discharge or increased pain, it is crucial to seek medical attention.
6. Hormonal Changes: Surgical interventions, especially those involving the breast, can lead to hormonal fluctuations. These changes can affect the skin and lead to sensations of itching.
7. Psychological Factors: Anxiety and stress related to the surgery and recovery process can manifest physically in various ways, including itching sensations.
When to Be Concerned
While itching is often benign, there are specific signs that warrant further evaluation:
- Persistent or Worsening Symptoms: If the itching does not improve or worsens over time, it is essential to consult with your healthcare provider.
- Accompanying Symptoms: If you experience additional symptoms such as swelling, redness, discharge, or changes in the appearance of the nipple or surrounding skin, these could indicate a more serious issue that needs to be addressed.
- Changes in Sensation: Any new or unusual sensations, such as pain or a burning feeling, should be discussed with your doctor.
Conclusion
Itchy nipples after surgery can be attributed to various benign causes, including the healing process, dry skin, or scar tissue formation. While it is natural to worry about potential signs of cancer, it is crucial to remember that not all changes in the body indicate malignancy. Regular follow-ups with your healthcare provider are essential for monitoring your recovery and addressing any concerns you may have. If the itching persists or is accompanied by other concerning symptoms, do not hesitate to seek further evaluation to ensure your peace of mind and health.
