Postoperative issues related to the ankle?

Hello Doctor: I have developed a cyst on my ankle, and I underwent surgery to remove it in September.
A month after the surgery, my ankle suddenly swelled up, but after a week, the swelling decreased somewhat.
However, as of yesterday, it suddenly swelled up again.
Could this be related to a previous ankle sprain, or is it connected to an incident six months ago when I bumped my calf (which I did not seek treatment for)? Thank you for your response, Doctor.

Niming, 30~39 year old female. Ask Date: 2017/11/06

Dr. Shi Guozheng reply Orthopedics

It is possible that the recurrence of a ganglion cyst after surgery is related to a sprain.
Continuous observation is sufficient, and if there is a recurrence, further management can be considered.

Post-surgery ankle swelling can be a concerning issue for many patients, especially after a procedure such as the removal of a ganglion cyst. It is essential to understand the potential causes of swelling and the factors that may contribute to it, particularly in the context of previous injuries.

Firstly, it is not uncommon for swelling to occur after surgery. This can be attributed to several factors, including inflammation, fluid accumulation, and the body’s natural healing response. After surgical procedures, the body often reacts by sending more blood and fluid to the area to promote healing, which can lead to swelling. In your case, the swelling that occurred one month post-surgery could be a normal part of the healing process, especially if it was accompanied by pain or discomfort.

However, the recurrence of swelling after a period of improvement raises some concerns. One possibility is that the swelling could be related to previous injuries, such as the ankle sprain or the impact injury to the calf you mentioned. Ankle sprains can lead to lingering instability and inflammation, which may be exacerbated by surgical trauma. If the ankle was previously injured, it might not have fully healed, and the added stress from the surgery could trigger a new inflammatory response.

Another factor to consider is the presence of scar tissue. Surgery can lead to the formation of scar tissue, which may restrict movement and cause discomfort. If the scar tissue is pressing on surrounding structures or if there is a buildup of fluid in the area, this could lead to swelling. Additionally, if the surgical site is not healing properly or if there is an infection, this could also result in increased swelling.

It is also important to consider the possibility of complications such as lymphedema, which is a condition that can occur when lymphatic fluid does not adequately drain from the affected area. This can happen after surgery, especially if lymph nodes were affected or if there was significant tissue disruption.
To address your concerns, it is crucial to monitor the swelling and any accompanying symptoms. If the swelling persists, worsens, or is accompanied by redness, warmth, or fever, it is essential to seek medical attention promptly. A healthcare professional may recommend imaging studies, such as an ultrasound or MRI, to assess for any underlying issues, including fluid accumulation or structural problems.

In the meantime, there are several self-care measures you can take to help manage the swelling. Elevating the affected ankle, applying ice packs, and using compression wraps can help reduce swelling and discomfort. Gentle range-of-motion exercises, as tolerated, can also promote circulation and aid in recovery.

In conclusion, while post-surgical swelling can be a normal part of the healing process, the recurrence of swelling after a period of improvement warrants further evaluation. Previous injuries may contribute to the swelling, and it is essential to consider all factors involved. Consulting with your healthcare provider will provide you with the best course of action tailored to your specific situation. They can help determine the underlying cause of the swelling and recommend appropriate treatment options to facilitate your recovery.
