Post-Retinal Tear Laser Surgery
Hello Dr.
Tsai, yesterday I underwent laser surgery for a retinal tear.
Last night, when I went to sleep and then opened my eyes after a while, I noticed a large dark shadow in the lower left corner of my vision, which felt like a visual afterimage.
However, when I looked directly at the shadow, it disappeared.
In bright spaces, I couldn't notice it, but after turning on the light in a previously dark room, the vision in the treated eye felt slightly narrowed for a moment before returning to normal.
Is this a normal occurrence during the recovery period after surgery, or should I go for a follow-up examination immediately?

Hello! Thank you for your question regarding the visual changes you are experiencing after your retinal hole laser surgery. It's understandable to feel concerned about any new visual symptoms following such a procedure.
Firstly, it's important to recognize that some visual disturbances can occur after retinal surgery, including laser treatment for retinal holes. These disturbances may include temporary changes in vision, such as the phenomenon you described—seeing a dark shadow or a visual "afterimage" when you first open your eyes. This can be attributed to several factors, including the healing process of the retina and the effects of the laser treatment itself.
1. Visual Afterimages: The black shadow you see in the lower corner of your vision when you first open your eyes may be a form of visual afterimage. This can happen as your eyes adjust to changes in light and focus after being closed for a period. The fact that it disappears when you look directly at it is a good sign, as it suggests that your retina is functioning properly in terms of detecting light and movement.
2. Temporary Visual Changes: The sensation of a reduced field of vision in dim lighting, which normalizes after a moment, can also be a common experience post-surgery. After laser treatment, the retina may take some time to stabilize, and fluctuations in vision can occur as the eye adjusts. This is particularly true in varying lighting conditions, where your pupils dilate or constrict, affecting how light enters the eye.
3. Healing Process: The healing process after retinal surgery can vary from person to person. During this time, it is not uncommon to experience visual disturbances, including blurriness, shadows, or even slight distortions. These symptoms often improve as the eye heals, but it is essential to monitor them closely.
4. When to Seek Help: While some visual changes can be expected, it is crucial to know when to seek further medical attention. If you experience any of the following, you should contact your eye care provider immediately:
- Sudden increase in the number of floaters or flashes of light.
- A significant change in vision, such as a sudden loss of vision or a curtain-like shadow.
- Persistent or worsening symptoms that do not improve over time.
5. Follow-Up Care: It is advisable to have a follow-up appointment with your ophthalmologist to discuss your symptoms and ensure that your recovery is progressing as expected. They can perform a thorough examination to rule out any complications, such as retinal detachment or other issues that may require intervention.
In conclusion, while some visual changes after laser surgery for a retinal hole can be normal, it is essential to remain vigilant and communicate any concerning symptoms to your eye care provider. They will be able to provide you with the best guidance based on your specific situation and ensure that your recovery is on track. Remember, your vision is precious, and timely follow-up care is key to maintaining it.
